超声引导下乙醇注射治疗莫顿神经瘤疗效观察

Effect of ultrasound-guided alcohol injection on Morton's neuroma

目的探讨超声引导下乙醇注射(ultrasound-guided alcohol injection,USGAI)治疗莫顿神经瘤的疗效。方法莫顿神经瘤患者47例(55个肿瘤),行USGAI治疗,对USGAI治疗4次后症状未缓解者行手术治疗;分别于USGAI治疗前、后行疼痛视觉模拟评分(visual analogue scale,VAS),记录不良反应的发生情况。结果 47例行USGAI治疗2~4次,其中2次12例,3次23例,4次7例,5例治疗4次后症状未缓解行手术治疗;USGAI治疗后VAS评分[(3.6±3.3)分]较治疗前[(8.7±1.3)分]降低(P<0.05);USGAI治疗过程中出现局部炎症反应2例,经对症治疗后缓解。结论USGAI可有效缓解莫顿神经瘤患者症状,可作为拒绝接受手术及手术高风险患者的替代治疗方法。

Objective To explore the effect of ultrasound-guided alcohol injection(USGAI)on Morton's neuroma.Methods Forty-seven patients(55Morton's neuromas)received USGAI treatment once to 4times,and the unrelieved patients after treatment 4times underwent operation.Visual analogue scale(VAS)scores were calculated before and after USGAI.The adverse reactions were recorded.Results Forty-seven patients received USGAI treatment twice to 4times,in which 12 patients received twice,23 patients received 3times,7patients received 4times,and 5patients failed after treatment 4times and underwent operation.VAS score was significantly lower after USGAI(3.6±3.3)than that before USGAI(8.7±1.3)(P<0.05).Two patients were found local inflammatory during USGAI,which was relieved after symptomatic treatment.Conclusion USGAI could relieve the symptoms of Morton's neuroma,and is a good alternative treatment for patients who refuse surgery or with high risk.
