多层螺旋CT三维血管重建技术诊断肺隔离症的价值 被引量:16

Value of multi-slice CT 3-deminsional vascular remodeling to the diagnosis of pulmonary sequestration

摘要 目的探讨多层螺旋CT三维血管重建技术在肺隔离症诊断中的价值。方法回顾性分析11例肺隔离症患者的多层螺旋CT增强扫描及多平面重建、最大密度投影、容积再现血管重建影像资料。结果 11例均为单发病灶,呈叶内型;病灶位于左肺下叶后基底段9例,右肺下叶后基底段2例;嚢性肿块5例,实质性肿块4例,混合性(嚢实性)肿块2例;隔离肺的供血动脉9例来自于胸降主动脉,2例来自于腹主动脉上段;异常供血动脉直径5~14 mm,其中5例供血动脉进入病灶后呈多分支状,包绕病灶边缘或伸入病灶内部;11例显示引流静脉均为肺静脉。结论多层螺旋CT三维血管重建技术可清晰显示肺隔离症异常供血动脉的起源、走行及其引流静脉,在肺隔离症的诊断中具有重要价值。 Objective To discuss the application value of 3-dimensional vascular remodeling by multi-slice CT to the diagnosis of pulmonary sequestration.Methods The imaging data as multi-slice CT contrast enhancement scanning results,and vascular remodeling data as multiple planar reconstruction,maximum intensity projection and volume rendering were retrospectively analyzed in 11 patients with pulmonary sequestration.Results All lesions of these 11 patients were solitary in intralobar type,in which 9 lesions were located at posterior basal segment of left lower lobe,and2 lesions were located at posterior basal segment of right lower lobe.There were 5 cases of cystic mass,4 cases of parenchymal mass and 2 cases of mixed lesions(cystic solid mass).The feeding artery of isolated pulmonary tissue was from the thoracic aorta in 9 cases,and from the upper abdominal aorta in 2 cases.The diameter of the artery was 5 to 14 mm,in which feeding artery showed multi-branch in lesions in 5 cases,surrounding the edge of the lesion or into the lesion.The drainage veins were indentified pulmonary vein in 11 cases.Conclusion Multi-slice CT 3-dimensional vascular remodeling can clearly display the origin and distribution of feeding vein,and drainage veins,playing an important role in the diagnosis of pulmonary sequestration.
