尿激酶型纤溶酶原激活物和细胞增殖核抗原Ki-67在胰腺癌组织中的表达

Expression of urokinase-type plasminogen activator and cell proliferation nuclear antigen Ki-67 in pancreatic cancer

目的探讨胰腺癌组织中尿激酶型纤溶酶原激活物(urokinase-type plasminogen activator,uPA)和细胞增殖核抗原Ki-67蛋白的表达情况。方法胰腺癌患者63例,取手术切除胰腺癌组织63份为胰腺癌组,癌旁非肿瘤胰腺组织11份为对照组,采用免疫组织化学法检测2组uPA和Ki-67蛋白的阳性表达情况,分析uPA、Ki-67蛋白阳性表达与临床病理特征的关系。结果胰腺癌组uPA和Ki-67蛋白表达阳性率分别为65.1%和74.6%,对照组表达均为阴性;胰腺癌组uPA蛋白阳性表达率在肿瘤分化程度、TNM分期、有无淋巴结转移上差异有统计学意义(P<0.05),Ki-67蛋白阳性表达率在肿瘤分化程度、TNM分期、有无淋巴结转移、有无远处转移上差异有统计学意义(P<0.05),uPA和Ki-67蛋白阳性表达在年龄、性别、肿瘤直径、肿瘤部位上差异无统计学意义(P>0.05);Spearman相关分析结果显示,uPA蛋白阳性表达与肿瘤分化程度呈负相关(r=-0.333,P=0.008),与TNM分期及淋巴结转移呈正相关(r=0.430,P<0.001;r=0.400,P=0.001);Ki-67蛋白阳性表达与肿瘤分化程度呈负相关(r=-0.333,P=0.008),与TNM分期、远处转移及淋巴结转移呈正相关(r=0.494,P<0.001;r=0.340,P=0.006;r=0.400,P=0.001);Spearman关联性分析结果显示,胰腺癌组织uPA和Ki-67蛋白阳性表达呈正关联(r=0.261,P=0.032)。结论 uPA和Ki-67蛋白阳性表达可能与胰腺癌的侵袭及转移有关,可作为胰腺癌诊断、分期及预后评估的参考指标之一。

Objective To explore the expressions of urokinase-type plasminogen activator(uPA)and cell proliferation nuclear antigen Ki-67 in pancreatic cancer.Methods Sixty-three copies of surgical removed pancreatic cancer tissue(cancer group)and 11 copies of adjacent non-tumor pancreatic tissue(control group)were detected the expressions of uPA and Ki-67 protein by immunohistochemistry and their relationship was analyzed.Results The positive rates of uPA and Ki-67 were 65.1% and 74.6% in cancer group,and both of uPA and Ki-67 were negatively expressed in control group.There were significant differences in the positive expression rates of uPA protein in different tumor differentiation degree and TNM stage,and between lymph node metastasis or not in cancer group(P<0.05).There were also significant differences in the positive expression rates of Ki-67 protein in different tumor differentiation degree and TNM stage,and between lymph node metastasis and distant metastasis or not(P<0.05).The uPA and Ki-67 protein positive rates showed no significant differences in the age,sex,tumor size and tumor location(P>0.05).Spearman correlation analysis showed the positive expression of uPA protein was negatively correlated with the degree of tumor differentiation(r=-0.333,P=0.008)and was positively correlated with TNM stage and lymph node metastasis(r=0.430,P<0.001;r=0.400,P=0.001),the positive expression of Ki-67 protein was negatively correlated with the degree of tumor differentiation(r=-0.333,P=0.008),and was positively correlated with TNM stage,distant metastasis and lymph node metastasis(r=0.494,P<0.001;r=0.340,P=0.006;r=0.400,P=0.001),and the expression of uPA was positively correlated with Ki-67(r=0.261,P=0.032).Conclusion The positive expressions of uPA and Ki-67 protein may be correlated with the invasion and metastasis of pancreatic cancer,which may be used as one of the reference indexes for the diagnosis,staging and evaluating the prognosis of pancreatic cancer.
