摘要
目的探讨类风湿关节炎(rheumatoid arthritis,RA)患者膝关节滑膜组织中Sirt2和Sirt3mRNA的表达情况及意义。方法行膝关节置换术的RA患者8例为RA组,行关节镜手术的半月板损伤患者6例为对照组,采用实时荧光定量PCR检测2组膝关节滑膜组织Sirt2、Sirt3mRNA表达水平,RA组患者检测抗环瓜氨酸多肽抗体、血沉、高敏C反应蛋白、类风湿因子水平,采用DAS28评分评估RA疾病活动度,采用Pearson相关性分析RA患者膝关节滑膜组织Sirt2、Sirt3mRNA表达水平与其临床检验指标间的相关性。结果 RA组膝关节滑膜组织Sirt2 mRNA表达水平(0.56±0.19)低于对照组(1.00±0.00),Sirt3mRNA表达水平(1.89±0.64)高于对照组(1.00±0.00)(P<0.05);RA患者抗环瓜氨酸多肽抗体水平为375.96(182.85,865.21)RU/mL,血沉47.00(28.25,68.25)mm/h,高敏C反应蛋白28.20(12.33,40.58)g/L,类风湿因子32.15(0,207.78)u/mL,DAS28评分4.18(3.52,4.69);RA患者膝关节滑膜组织Sirt3mRNA表达水平与抗环瓜氨酸多肽抗体呈正相关(r=0.770,P=0.025),与血沉、高敏C反应蛋白、DAS28评分、类风湿因子和病程无相关性(P>0.05),RA患者膝关节滑膜组织Sirt2mRNA表达水平与临床各指标均无明显相关性(P>0.05)。结论 RA患者膝关节滑膜组织中Sirt3mRNA表达增高,Sirt2mRNA表达降低;Sirt3可能与RA疾病预后相关。
Objective To explore the expressions and significances of Sirt2 and Sirt3 mRNA in synovial tissues of knee joint in patients with rheumatoid arthritis(RA).Methods The expressions of Sirt2 and Sirt3 mRNA in synovial tissues of knee joint were detected by real-time quantitative PCR in 8 patients with RA undergoing joint replacement(RA group)and 6 patients with menisici injuries undergoing arthroscopic surgery(control group).The levels of anti-cyclic peptide containing citrulline(anti-CCP)antibody,erythrocyte sedimentation rate(ESR),high-sensitivity C-reactive protein(hsCRP)and rheumatoid factor(RF)were detected in RA group.RA disease activity was analyzed by Disease Activity Score28(DAS28).The correlations of clinical indexes with Sirt2 and Sirt3 mRNA were analyzed by Pearson correlation method.Results The level of Sirt2 mRNA was significantly lower,and the level of Sirt3 mRNA was significantly higher in RA group(0.56±0.19,1.89±0.64)than that in control group(1.00±0.00,1.00±0.00)(P<0.05).In RA group,anti-CCP antibody was 375.96(182.85,865.21)RU/mL,ESR was 47.00(28.25,68.25)mm/h,hs-CRP was28.20(12.33,40.58)g/L,RF was 32.15(0,207.78)IU/mL and DAS28 was 4.18(3.52,4.69).The level of Sirt3 mRNA expression in RA group was positively correlated with anti-CCP antibody(r=0.770,P=0.025),and was not correlated with ESR,hs-CRP,DAS28,RF and disease duration(P>0.05).The level of Sirt2 mRNA was not correlated with all above clinical indexes(P>0.05).Conclusion Sirt3 mRNA expression increases and Sirt2 mRNA expression decreases in synovial tissues of knee joint in RA patients.The level of Sirt3 may be correlated with the prognosis of RA.
