中医药在甲状腺癌术后的应用

Application of Chinese Medicine in Thyroid Cancer after Surgery

甲状腺癌术后证候虚实夹杂,早期以实证居多,中晚期以虚证居多。中医药在辨证论治的基础上,总结出甲状腺癌术后8种中医常见证型,并分别给予不同的治疗方法:如风热犯表证,治以疏风清热解表;肝火旺盛证,治以清泻肝胆实火,软坚散结;气郁痰阻证,治以疏肝解郁,化痰散结;气滞血瘀证,治以活血化瘀散结,理气止痛;气阴两虚证,治以益气养阴,生津润咽,破血消癥;气血两虚证,治以益气补血;阴阳两虚证,治以阴阳双补;瘀毒阻滞证,治以化瘀解毒,破血散结消癥。一些中成药如玉龙胶囊、金龙胶囊等治疗本病临床效果较好,但如不加以辨证的"盲用""滥用",疗效也不尽人意。只有做到因人制宜,以辨证论治为宗旨,甲状腺癌术后的中医药治疗才会取得最佳疗效。

Postoperative syndromes of thyroid cancer are mixed ones. Most of them are excessive syndromes in the early stage,and most of them are deficiency syndromes in the middle and late stages. On the basis of TCM syndrome differentiation and treatment,8 common syndromes of TCM after thyroid cancer surgery was summarized,and different treatment methods were used respectively.With Wind-heat offending the exterior,the Wind and Heat should be cleard and the exterior syndrome should be relieved. With Liver Fire syndrome,Liver and Gallbladder Fire should be cleared and resolve the stasis. With Qi stagnation and Phlegm Syndrome,method of soothing Live Qi and expelling stasis should be used. With Qi stagnation and Blood stasis syndrome,treatment of promoting blood circulation as well as regulating Qi and stop pain should be applied. With Qi and Yin deficiency syndrome,treatment of tonifying Qi and Yin as well as eliminating blood stasis should be applied. With Qi and Blood deficiency syndrome,treatment of tonifying Qi and Blood should be used. With Yin and Yang deficiency syndrome,treatment of tonifying Yin and Yang should be applied. With Stasis and Toxin blocking syndrome,treatment of dissolving Phlegm and Detoxification as well as eliminating Blood stasis should be used. Some Chinese patent medicines such as Yulong Capsule and Jinlong Capsule have significant clinical effects in treating for this disease. However,if they are blindly used without syndrome differentiation,the clinical effect cannot be achieved. Therefore,only by doing it according to patient’ physical conditions and with the applying of the principle of syndrome differentiation and treatment,the treatment of thyroid cancer after surgery would achieve the best effect.
