肾衰合剂在经皮肾镜碎石术中的应用

Application of Shenshuai Compound Medicine in Percutaneous Nephrolithotomy

目的:探讨肾衰合剂在经皮肾镜碎石术中的作用。方法:50例上尿路结石患者随机平均分为对照组和观察组。对照组常规行经皮肾镜碎石术;观察组在术前3 d及术后8 h开始口服肾衰合剂。比较两组患者术前及术后第1天、2天、3天患肾尿蛋白与尿肌酐比值、手术时间、术后血红蛋白减少值、术后住院时间、结石清除率和术后高热发生率。结果:观察组在术后第1天、2天、3天的肾尿蛋白与尿肌酐比值均显著低于对照组,差异均有统计学意义(P <0. 05)。观察组手术时间显著短于对照组,差异有统计学意义(P <0. 05)。观察组术后血红蛋白减少值显著低于对照组,差异有统计学意义(P <0. 05)。对照组术后高热发生率为32%,观察组为8%,两组比较,差异有统计学意义(P <0. 05)。对照组结石清除率为84%,观察组为92%,两组比较,差异有统计学意义(P <0. 05)。结论:肾衰合剂用于经皮肾镜碎石术可以保护肾单位,促进肾功能恢复,提高结石清除率,减少血红蛋白的下降,降低术后高热发生概率。

Objective: To investigate the role of Shenshuai Compound medicine in percutaneous nephrolithotomy. Methods: Fifty patients with upper urinary calculi were randomly divided into control group and observation group. The control group underwent routine percutaneous nephrolithotomy;the observation group started to use oral Shenshuai Compound at 3 days before surgery and 8hours after surgery. The ratio of renaluria protein to urine creatinine,operative time,postoperative hemoglobin reduction,postoperative hospital stay,stone clearance rate,and postoperative hyperthermia were compared between the two groups before and after the first,second,and third days after surgery. Results: The ratio of renaluria protein to urinary creatinine in the observation group was significantly lower than that in the control group on the 1st,2nd,and 3rd postoperatively( P < 0. 05). The operation time of the observation group was significantly shorter than that of the control group( P < 0. 05). The hemoglobin reduction value of the observation group was significantly lower than that of the control group( P < 0. 05). The incidence of postoperative hyperthermia was 32%in the control group and that was 8% in the observation group. The difference was statistically significant( P < 0. 05). The stone removal rate was 84% in the control group and that was 92% in the observation group. The difference was statistically significant( P < 0. 05). Conclusion: Shenshuai Compound combined with percutaneous nephrolithotomy can protect the nephron,promote the recovery of renal function,improve the stone clearance rate,reduce the decrease of hemoglobin,and reduce the incidence of postoperative hyperthermia.
