Discussion on Medicating Characteristics of Zhang Xichun in Treatment of Epilepsy

张锡纯治疗癫痫,首提痰火致痫的思想,认为因顽痰瘀阻,肝经风火挟顽痰上行,导致心脑隔离,出现神志异常等症状。治疗上,张锡纯遵循'急则治其标,缓则治其本'的原则。发作期坠痰镇惊以治标,药用代赭石、磁石、铅灰、硫化铅、朱砂、铁锈等矿物质坠痰镇惊;开痰泻热以治本,配伍有开痰散结之用的半夏、朴硝、芒硝、郁金、甘遂;并兼以顾护脾胃,多佐以神曲、麦曲、酒曲、甘松等醒脾开胃,顾护中焦。静止期调气养神护中以善后。治疗时亦可采用中西结合疗法,用西药治其标以控制癫痫发作,发作频繁可加大西药使用量。但西药不能拔出病根,待到患者身体强壮,即可停止西药,用镇惊坠痰、开痰泻热、调神护中之中药,病根自能拔出无余。

Zhang Xichun,in treating epilepsy,first proposed the thought of Phlegm inducing epilepsy,by believing that when the stasis and resistance of the heavy Phlegm and Fire,they are carried and led up human body by the Liver Wind and Fire,which would cause the disharmony between human heart and brain,and the abnormal symptoms in spirit would take place.In terms of treatment,Zhang Xichun follows the principle of'immediately treating its symptoms under critical situation and patiently treating the root of the disease under if situation is not urgent'.Specifically,in the attack period,the treatment should be carried out with dispelling Phlegm and relieving convulsion in order to eliminate the symptoms,by using minerals such as Hematite Haematitum,magnetite,Lead ash,Lead sulfide,Cinnabar and Rust on the one hand;the treatment should be carried out with dispelling Phlegm and purging Heat to cure the root of the disease on the other hand,with drugs such as Pinellia,Park Nitro,Glauber’s salt,Turmeric and Euphorbia kansui.At the same time,the Spleen and Stomach should be taken care of by using Medicated leaven,wheat koji,distiller’s yeast,Nardostachys chinensis and others.During the quiescent period,the Qi is regulated and the spirit is protected to improve health level of the patients.The TCM treatment can also be combined with Western therapy to control the break out of the disease.With frequent attacks the dose of Western medicine can be increased.However,western medicine can’t pull out the root of the disease.Therefore,when the patient is strong enough,the western medicine should be stopped.By using the TCM treatment mentioned above,the disease can be cured completely.
