中医辨证疗法对艾滋病抗病毒治疗后肝损伤的影响

Effect of TCM Syndrome Differentiation on Liver Injury after AIDS Antiviral Treatment

目的:观察中医辨证疗法治疗艾滋病抗病毒治疗后肝损伤的影响。方法:60例艾滋病抗病毒治疗后发生肝损伤的患者作为研究对象,随机分为对照组和观察组各30例。对照组患者给予西药还原型谷胱甘肽片,观察组患者给予中医辨证疗法,4周后比较两组患者临床疗效。结果:治疗后,两组患者肝功能各项指标均明显降低,CD4+水平显著升高(P<0.05);观察组患者天冬氨酸氨基转移酶水平为(65.8±26.4)U·L-1,丙氨酸氨基转移酶水平为(79.3±23.8)U·L-1,血清总胆红素水平为(22.5±13.3)mmol·L-1,直接胆红素水平为(12.9±9.3)mmol·L-1,均显著低于同期对照组患者(t=7.875,6.224,8.136,6.154,P<0.05);而CD4+水平为(349.6 6±203.4)copy·mm-3,显著高于对照组患者(t=9.115,P<0.05)。治疗后,两组患者经过中医各证候积分均发生了下降,其中观察组患者治疗后总分为388分,降低率为29.58%,显著高于对照组(χ2=4.335,P<0.05)。观察组有效率为96.7%,显著高于对照组的86.7%(χ2=5.872,P<0.05)。结论:中医辨证疗法对艾滋病抗病毒治疗后肝损伤患者有确切疗效,可显著改善患者肝功能,降低患者中医证候积分。

Objective:To observe the effect of TCM syndrome differentiation therapy on liver injury after AIDS antiviral therapy.Methods:Sixty patients with liver injury after AIDS antiviral therapy were randomly divided into control group and observation group.The control group received Western medicine reduced glutathione tablets,the observation group patients were given TCM syndrome differentiation therapy,and the clinical efficacy of the two groups was compared after 4 weeks.RESULTS:After treatment,the liver function indexes of the two groups were significantly decreased,and the CD4+level was significantly increased(P<0.05).The level of aspartate aminotransferase was(65.8±26.4)U·L-1 in the observation group.The level of alanine aminotransferase was(79.3±23.8)U·L-1,the serum total bilirubin level was(22.5±13.3)mmol·L-1,and the direct bilirubin level was(12.9±9.3)mmol·L-1 was significantly lower than the control group(t=7.875,6.224,8.136,6.154,P<0.05);and CD4+level was(349.6 6±203.4)copy·mm-3,significantly higher than the control group Patients(t=9.115,P<0.05).After treatment,the scores of the syndromes of the two groups were decreased.The total score of the patients in the observation group was 388,and the reduction rate was 29.58%,which was significantly higher than that of the control group(χ2=4.335,P<0.05).The effective rate of the observation group was 96.7%,which was significantly higher than 86.7%of the control group(χ2=5.872,P<0.05).Conclusion:TCM syndrome differentiation therapy has a definite curative effect on patients with liver injury after AIDS antiviral therapy,which can significantly improve liver function and reduce TCM syndrome scores.
