中西医结合治疗灼口综合征

Combination of Chinese and Western Medicine in the Treatment of Burning Mouth Syndrome

目的:探讨中西医结合治疗灼口综合征(burning mouth syndrome,BMS)的临床疗效。方法:将明确诊断为BMS的患者386例,随机分成观察组和对照组,每组193例。观察组口服增液汤合清心莲子饮,并联合维生素B1、B12口服及双侧下颌神经封闭治疗;对照组采用21金维他、维生素B1、Fe制剂、甲钴铵片、雌激素、黄体酮,并联合复方氯己啶(漱口)、抗抑郁等常规西药综合治疗。两组患者均常规给予心理疏导,疗程均为6周。结果:治疗3周、6周及疗程结束后3个月复查,观察组疼痛指数均较对照组明显减小,差异有统计学意义(P<0.01)。治疗3周后,观察组疼痛指数较治疗前明显减小,差异有统计学意义(P<0.05);对照组较治疗前也有所减小,但差异无统计学意义(P>0.05)。治疗6周后,观察组疼痛指数较治疗前、治疗3周均有所改善,差异有统计学意义(P<0.01);对照组疼痛指数较治疗前减小(P<0.05),但与治疗3周后比较,差异无统计学意义(P>0.05)。治疗结束后3个月,观察组疼痛指数较治疗前、治疗3周均明显减小,差异有统计学意义(P<0.01),与治疗6周时比较,差异无统计学意义(P>0.05);对照组与治疗3周、6周时比较,差异均无统计学意义(P>0.05)。观察组有效率明显优于对照组(P<0.01)。观察组患者舌乳头、舌苔恢复情况、唾液分泌量、女性雌激素水平均明显优于对照组。结论:中西医结合治疗BMS疗效显著,起效快,不易复发。

Objective:To explore the clinical efficacy of combined of Chinese and western medicine in the treatment of burning mouth syndrome(BMS).Methods:386 patients with BMS were randomly divided into observation group and control group,193 cases in each group.The observation group took Zengye Decoction and Qingxin Lianzi Yin orally,and combined with vitamin B1,B12 oral and bilateral mandibular nerve block treatment;the control group used 21 golden vitamins,vitamin B1,Fe preparation,methylcobalt ammonium tablets,estrogen,progesterone,and combined with conventional western medicine such as compound chlorhexidine(gargle),antidepressant treatment.Both groups were given psychological counseling routinely for 6 weeks.Results:After 3 weeks,6 weeks and 3 months of treatment,the pain index in the observation group was significantly lower than that in the control group.The difference was significant(P<0.01).After 3 weeks of treatment,the pain index in the observation group was significantly lower than that before treatment.The difference was statistically significant(P<0.05);the control group also decreased compared with before treatment,but there was no significant difference(P>0.05).After 6 weeks of treatment,the pain index of the observation group was improved compared with that before treatment and 3 weeks after treatment,and the difference was statistically significant(P<0.01);the pain index in the control group was lower than that before treatment(P<0.05),but there was no significant difference between the two groups after 3 weeks of treatment(P>0.05).Three months after the end of treatment,the pain index in the observation group was significantly lower than that before treatment and three weeks after treatment(P<0.01),and there was no significant difference between the two groups(P>0.05);there was no significant difference between the control group and the treatment group at 3 and 6 weeks(P>0.05).The effective rate of the observation group was significantly better than that of the control group(P<0.01).The recovery of tongue papilla,tongue coating,salivary secretion and estrogen level in the observation group were significantly better than those in the control group.Conclusion:Combination of Chinese and western medicine in the treatment of BMS has remarkable curative effect,quick onset and no recurrence.
