生物反馈联合TENS治疗女性慢性盆腔疼痛综合征的疗效分析 被引量:6

Analysis of efficacy of biofeedback combined with TENS in treatment of chronic pelvic pain syndrome

摘要 目的探讨生物反馈联合经皮神经电刺激(TENS)治疗女性慢性盆腔疼痛综合征的临床效果。方法选取2014年6月至2017年6月郑州大学第三附属医院收治的80例诊断为慢性盆腔疼痛综合征(CPPS)患者,随机分为TENS组(n=40)和生物反馈联合TENS治疗组(联合组,n=40),TENS组进行经皮神经电刺激治疗2个月;联合组经皮神经电刺激治疗2个月后进行盆底生物反馈治疗2个月。观察和记录治疗前、治疗后1个月的盆底肌电情况、盆腔疼痛评分(VAS),以及治疗后6个月复发情况。结果 (1)联合组治疗后盆底肌电值及I、II类肌纤维肌电值均较前明显提高,有统计学意义(P <0. 05);而TENS组肌电值治疗前后差异无统计学意义(P> 0. 05);(2)两组患者治疗后VAS评分与治疗前比较差异有统计学意义(P <0. 01),但治疗前后两组VAS评分改变量比较无统计学意义(P> 0. 05);(3)联合组治疗后显效率及总有效率(72. 5%,97. 5%)明显高于TENS组(57. 5%,92. 5%);治疗后1个月两者复发率差异无统计学意义(P> 0. 05),而治疗后6个月联合组复发率(2. 5%)较TENS组(7. 5%)明显下降。结论生物反馈联合TENS能够有效缓解女性慢性盆腔痛症状且具有较好的远期效应。 Objective To investigate the clinical effects of biofeedback combined with TENS in the treatment of chronic pelvic pain syndrome in women.Methods A total of 80 patients diagnosed with chronic pelvic pain syndrome(CPPs)were randomly divided into the TENS treatment group(n=40)and the biofeedback combined with TENS treatment group(n=40),TENS group underwent transcutaneous electrical nerve stimulation for 2 months;The combined treatment group received pelvic floor biofeedback for 2 months after 2 months of transcutaneous electrical nerve stimulation.The pelvic floor electromyography,pelvic pain score(VAS),and recurrence at 6 months after treatment were observed and recorded before treatment and 1 month after treatment.Results(1)After combined treatment,the myoelectric value of pelvic floor and I and II muscle fibers myoelectric were significantly increased,which was statistically significant;There was no significant difference of the myoelectric value in the TENS group before and after the TENS treatment.(2)The VAS score was significantly improved in both groups after treatment compared with before treatment(P<0.01)which was statistically significant,but the VAS score of the two groups before and after treatment was not statistically significant.(3)Significant efficiency and total efficiency were significantly higher in the combined group than those in the TENS group.There was no significant difference in the recurrence rate of both groups at 1 month after treatment,while the recurrence rate of the combined group at 6 months after treatment was significantly lower than that of the TENS group.Conclusion The combination of biofeedback and TENS can effectively alleviate the symptoms of chronic pelvic pain in women and has a better long-term effect.
