新生儿末梢血与静脉血血常规检验结果对比研究

Comparative study of routine examination of neonatal peripheral blood and venous blood

目的分析新生儿末梢血与静脉血在血常规检测中的差异以及在临床诊断中的意义。方法将2016年1月—2018年1月在郑州颐和医院出生的80例足月健康新生儿随机分为静脉血组与末梢血组各40例,分别检测白细胞计数(WBC)、血小板计数(PLT)、红细胞计数(RBC)、红细胞压积(Hc T)、不同时间检测结果、诊断满意率。结果检验完成后末梢血组在WBC和RBC水平上高于静脉血组,而PLT水平低于静脉血组,差异有统计学意义(P <0. 05),两组Hc T水平差异无统计学意义(P> 0. 05);两组受检新生儿的不同时间段检测结果分析,静脉血组各时间段的血小板与白细胞水平差异无统计学意义(P> 0. 05);末梢血组0 min血小板水平与10 min、30 min比较差异有统计学意义(P <0. 05),而三个时间段白细胞水平呈递减,差异有统计学意义(P <0. 05)。结论静脉血与末梢血的检测结果具有一定的差异,末梢血并不能完全反应全身循环系统血液的真实情况,在临床检验当中,建议根据新生儿的具体情况选择检测标本。

Objective To analyze the differences between neonatal peripheral blood and venous blood in routine blood test and the significance in clinical diagnosis.Methods Totally 80 healthy full-term neonates born in our hospital from January 2016 to January 2018 were randomly divided into the venous blood group and the peripheral blood group(40 cases each),and the white blood cell count(WBC),platelet count(PLT),RBC count(RBC),HcT,different time test results and diagnostic satisfaction rate were detected.Results After the examination,the peripheral blood group was higher in the level of WBC and RBC than the venous blood group,while the PLT level was lower than that of the venous blood group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the HcT level between the two groups(P>0.05).Analysis of the test results of the neonates in the two groups at different time periods showed no significant difference in platelet and leukocyte levels in the venous blood group(P>0.05).The platelet level of the peripheral blood group at 0min was statistically significant compared with that at 10 min and 30min(P<0.05),while the white blood cell level was decreasing at three time periods and the difference was statistically significant(P<0.05).Conclusion There is a certain difference between the detection results of venous blood and peripheral blood.Peripheral blood cannot fully reflect the real situation of blood in systemic circulation system.In clinical examination,it is suggested to select the sample according to the specific situation of the newborn.
