经前外侧入路微创小切口全髋关节置换术治疗股骨颈骨折患者的短期临床研究

Short-term clinical study of minimally invasive total hip arthroplasty via anterolateral approach for femoral neck fractures

目的观察经前外侧入路微创小切口全髋关节置换术治疗股骨颈骨折患者的短期临床效果。方法选取我院2016年1月—2018年4月股骨颈骨折患者73例,抽签法分组,对照组36例采取经后外侧入路全髋关节置换术治疗,研究组37例采取经前外侧入路微创小切口全髋关节置换术治疗,比较2组术中失血量、手术用时、住院用时及髋关节功能。结果研究组术中失血量较对照组少,手术用时、住院用时较对照组短(P<0.05);2组末次随访Harris评分显著升高,且研究组Harris评分较对照组高(P<0.05)。结论股骨颈骨折患者采取经前外侧入路微创小切口全髋关节置换术治疗,能减少术中失血,缩短手术及住院用时,提升髋关节功能。

Objective To observe the short-term clinical effect of minimally invasive total hip arthroplasty via anterolateral approach in the treatment of femoral neck fracture.Methods 73 patients with femoral neck fracture in our hospital from January 2016 to April 2018 were divided into groups by lottery.36 cases in the control group were treated with total hip arthroplasty via posterolateral approach,and 37 cases in the study group were treated with minimally invasive total hip arthroplasty via anterolateral approach.The intraoperative blood loss,operative time,hospitalization time and hip joint function were compared between the 2 groups.Results The blood loss in the study group was less than that in the control group,and the operation time and hospitalization time were shorter than those in the control group(P<0.05);The Harris score of the 2 groups was significantly higher at the last follow-up,and the Harris score of the study group was higher than that of the control group(P<0.05).Conclusion Patients with femoral neck fracture treated by minimally invasive total hip arthroplasty through anterolateral approach can reduce intraoperative blood loss,shorten operation and hospital stay,and improve hip function.
