药物联合宫腔镜治疗早期未破裂型宫角妊娠疗效观察

Curative effect of drugs combined with hysteroscopy in treatment of early unruptured cornual pregnancy

目的分析早期未破裂型宫角妊娠采用药物联合宫腔镜治疗的临床疗效。方法收集2011年6月—2016年3月驻马店市第一人民医院收治的早期未破裂型宫角妊娠患者40例,所有患者均得到确诊,采用药物(米索前列醇片+米非司酮)联合宫腔镜治疗,分析临床治疗效果。结果 40例患者中,有32例患者经药物治疗5~6 h后,孕囊自行排出,还有8例患者未见孕囊排出;所有患者在宫腔镜定位之后均行吸宫术治疗,均一次性吸宫干净,阴道出血在10~40 ml;人绒毛膜促性腺激素(β-HCG)值在术后1周~4周之间恢复正常;术后对患者行13~27个月的随访,发现其月经恢复正常的时间在35~42 d,且有16例患者成功妊娠分娩。结论早期未破裂型宫角妊娠采用药物联合宫腔镜治疗的临床疗效显著,安全性高。

Objective to analyze the clinical efficacy of drug combined hysteroscopy in the treatment of early unruptured cornual pregnancy.Methods a total of 40 cases of early unruptured horn pregnancy admitted to our hospital from June 2011 to March 2016 were collected.All patients were diagnosed and treated with drug(misoprostol tablets+mifepristone)combined with hysteroscopy to analyze the clinical effect.Results among the 40 patients,32 patients were discharged spontaneously after 5~6 h of drug treatment,and 8 patients were not discharged.All the patients were treated with uterine aspiration after hysteroscopic positioning.All the patients had a clean uterine aspiration and the vaginal bleeding was between 10 and 40 ml.Human chorionic gonadotropin(-hcg)levels returned to normal between 1 and 4 weeks after surgery.Postoperative follow-up of the patients for 13 to 27 months showed that the recovery time of menstruation was between 35 and 42 d,and 16 patients had a successful pregnancy and delivery.Conclusion Early unruptured uterine horn pregnancy with drugs combined with hysteroscopy treatment of clinical efficacy is significant,high safety.
