摘要
目的探讨新生儿溶血三项试验检测数据,为该地区新生儿溶血病(hemolytic disease of the new born,HDN)的诊治提供依据。方法采用微柱凝胶法对整群选取的该院879例新生儿高胆红素血症的患儿进行AB0、RhD血型鉴定及新生儿溶血三项(直接抗人球蛋白试验、血清游离抗体试验、抗体释放试验)检测,各组间率的比较使用SPSS17.0软件包,各血型HDN检出率HDN阳性分布的比较采用χ2检验,P<0.05为差异有统计学意义。结果送检样本中,A、B、0、AB血型阳性率为23.34%、20.46%、0.00%和3.77%,;Rh-HDN阳性率依次为0.70%、1.16%、4.64%和1.89%,组中各血型阳性率分布差异有统计学意义(P<0.05);所有AB0-HDN阳性样本中,A、B、0、AB血型比例分别为54.92%、43.44%、0.00%和1.64%,Rh-HDN中各血型阳性率分别为10.53%、15.79%、68.42%和5.26%,HDN确诊患儿中血型分布差异有统计学意义(P<0.05);结论 HDN主要以AB0血型不合引起,Rh-HDN少见;ABO-HDN中以A、B型血新生儿为主,Rh-HDN中则多见于O型血新生儿。
Objective To investigate the values of the three tests of hemolytic disease of the newborn(HDN) so as to provide reference for the diagnosis and treatment of the disease in the local district. Methods Using the micro column gel method to test ABO, RhD blood grouping and hemolytic three(direct Coombs test, serum free antibody test, antibody release test) of 879 children with neonatal hyperbilirubinemia in the hospital.Compared rates between groups using SPSS17.0 package, and compared the relevance ratio of HDN and HDN positive distribution of each blood type using χ2test, and P <0.05 was considered statistically significant. Results Within the submission samples, positive rate of A, B, O, AB blood type was 23.34%, 20.46%, 0.00% and3.77% respectively. Rh-HDN-positive rate was 0.70%, 1.16%, 4.64% and 1.89% respectively. The distribution of positive rate of each blood type group had statistically significant differences(P <0.05); All ABO-HDN positive samples, A, B, O, AB blood type ratio was 54.92%, 43.44%, 0.00% and 1.64% respectively, and in each blood type Rh-HDN positive rates was 10.53%, 15.79%,68.42% and 5.26% respectively,and blood type distribution of children diagnosed as HDN had statistically significant differences(P <0.05). Conclusion HDN caused mainly by ABO blood type incompatibility, Rh-HDN rare; and ABO-HDN was mainly occurred in newborns with A or B-type blood, while Rh-HDN is more common in newborns with O blood type.
