经皮病灶清除联合局部化疗治疗脊柱结核患者的临床研究

Clinical Research of Percutaneous Focal Debridement Combined with Local Chemotherapy in Patients with Spinal Tuberculosis

目的探讨经皮病灶清除联合局部化疗治疗脊柱结核患者的临床疗效。方法将整群选取该院2011年8月—2014年8月期间收治的64例脊柱结核患者随机均分为观察组和对照组,对照组采用传统开放病灶清除术进行治疗,观察组采用经皮病灶清除术联合局部化疗进行治疗。观察两组临床效果。结果观察组治疗总有效率为96.8%,明显大于对照组的75%,差异有统计学意义(P<0.05);观察组术后ESR、CRP明显低于对照组,差异有统计学意义(P<0.05);观察组术中出血量、手术时间、病椎愈合时间均优于对照组,差异有统计学意义(P<0.05)。结论经皮病灶清除联合局部化疗治疗脊柱结核患者有助于减少术中出血量、手术及病椎愈合时间、缓解患者痛苦,可在临床上推广应用。

Objective To explore the clinical effect of percutaneous focal debridement combined with local chemotherapy in the treatment of patients with spinal tuberculosis. Methods 64 patients with spinal tuberculosis were admitted to our hospital between August 2011 and August 2014. We randomly divided them into control group(n =32, undergoing traditional open debridement),and observation group(n=32, receiving percutaneous focal debridement combined with local chemotherapy). The clinical effects of the two groups were compared. Results The total effective rate of the observation group was 96.8%, significantly higher than that of the control group(75%),and the difference was statistically significant(P<0.05); the ESR and CRP after operation of the observation group were significantly lower than those of the control group, and the differences were statistically significant(P<0.05); the amount of bleeding, operation time and disease healing time of the observation group were less than those of the control group, and the differences were statistically significant(P<0.05).Conclusion Percutaneous focal debridement combined with local chemotherapy in the treatment of patients with spinal tuberculosis can reduce the amount of bleeding, shorten the operation duration and disease healing time, and relief their pain, therefore this method could be widely applied in clinical practice.
