冠状动脉旁路移植术同期行心脏瓣膜手术治疗冠心病合并心脏瓣膜病变的临床分析

Coronary Artery Bypass Line During the Same Period of Heart Valve Surgery Clinical Analysis for the Treatment of Coronary Heart Disease with Heart Valve Disease

目的探讨冠状动脉旁路移植术同期行心脏瓣膜手术治疗冠心病合并心脏瓣膜病变的临床价值。方法整群收集该院2013年4月—2015年4月心脏外科行冠状动脉旁路移植术同期进行心脏瓣膜手术的病例82例,收集术后相关心脏功能状态指标分析、术后相关并发症症情况。结果心胸比、左心室舒张末期前后径(LVD A-P)、左心房收缩末期前后径(LAS A-P)较术前明显降低(P<0.05),而左室射血分数较术前也降低(P<0.05);冠状动脉旁路移植术同期行心脏瓣膜手术术后并发低心排综合症3例、肝功能不全2例、肾功能不全4例、肺功能不全5例、胸腔积液12例、二次开胸2例、死亡2例。结论冠状动脉旁路移植术同期行心脏瓣膜手术治疗冠心病合并心脏瓣膜病变具有较为确切的临床有效性,值得临床推广。

Objective To investigate the coronary artery bypass line during the same period of heart valve surgery in the treatment of coronary heart disease with heart valve disease value. Methods Collected in our hospital during April 2015-04, 2013 in our hospital cardiac surgery coronary artery bypass transplantation cases of heart valve surgery in 82 cases during this period, the collection of postoperative cardiac function index analysis, related to postoperative complications. Results Before and after cardiothoracic ratio and left ventricular end-diastolic diameter(LVD A- P), left atrial systolic diameter(LAS A-P) before and after the end of the preoperative significantly decreased(P < 0.05), and left ventricular ejection fraction compared with preoperative also lower(P < 0.05); Coronary artery bypass line during the same period of heart valve surgery complicated with low cardiac syndrome(3cases), hepatic insufficiency in 2 cases, renal insufficiency in 4 cases, 5 cases of pulmonary insufficiency, 12 cases pleural effu-sion, secondary thoracotomy in 2 cases, 2 cases died. Conclusion Coronary artery bypass grafting in the same period underwent surgical treatment of coronary heart disease with heart valve heart valve disease with a more exact clinical effectiveness, in this pa-per, the experimental results that this method effectively only, if it is concluded that the results of the comparative significance,and should set controlled trials, with conventional treatments for the statistical analysis of related indicators, to prove that this arti-cle conclusion] worth clinical promotion.
