等离子鼻内窥镜下治疗高原鼻出血的效果研究

Effect Research of Plateau Nasal Bleeding Cured by Plasma Technology with Nasal Endoscope

目的探讨等离子鼻内窥镜下治疗高原鼻出血的疗效。方法整群选取2013年2月—2015年6月该院收治的1680例鼻出血患者,通过表面麻醉,经鼻内窥镜寻找并明确鼻内出血部位,然后采用等离子技术进行止血治疗,并分析其出血部位,研究其治疗效果。结果 1680例高原鼻出血患者,出血部位分别为鼻中隔前下方、嗅裂鼻中隔部、下鼻道后端顶部、下鼻道、下鼻甲,人数对应为1265、235、157、12、11,比例分别为75.29%、13.99%、9.34%、0.71%、0.71%,显然高原鼻出血的多发部位仍是鼻中隔前下方。患者全部采用等离子鼻内窥镜下治疗,其中一次治愈1617例(96.25%),63例(3.75%)出现二次出血,经2次等离子治愈,治愈效率100%。结论鼻内窥镜下等离子治疗高原鼻出血具有定位准确、创面小、快速止血、愈合质量好、并发症少、无辐射、安全程度高等优点,可作为今后治疗高原鼻出血的首选方法。

Objective To probe into the clinical effects of plateau nasal bleeding cured by plasma technology with nasal endoscope. Methods 1680 cases with nasal bleeding admitted in our department from February 2013 to June 2015 were selected. Under superficial anesthesia, the nasal bleeding sites of the patients were found and made clear by nasal endoscope and plasma technology was used for hemostatic therapy. The bleeding sites were analyzed. And the therapeutic effect was studied. Results The bleeding sites of the 1680 cases with plateau nasal bleeding included anterior inferior part of nasal septum [1265 cases(75.29%)], olfactory cleft nasal septum [235 cases(13.99%)], top of lower nasal end [157 cases(9.34%)], inferior nasal passage [12 cases(0.71%)], inferior turbinate [11 cases(0.71%)], which showed that the predilection site of plateau nasal bleeding was anterior inferior part of nasal septum. All the patients were treated by plasma technology with nasal endoscope. 1617 cases(96.25%) were cured by one time. 63 cases(3.75%) had secondary hemorrhage, but cured by second plasma technology treatment. The cure rate was 100%. Conclusion For the treatment of plateau nasal bleeding,plasma technology with nasal endoscope has the advantages of accurate positioning, small wound, rapid hemostasis, good healing quality, less complications, no radiation and high safety, which can be used as the first choice for the treatment of plateau nasal bleeding in the future.
