摘要
目的探讨静息态fMRI在脑功能皮质定位及脑功能重塑的应用方式。方法方便选取该院收治的18例主侧板球脑胶质瘤手术及22例全臂丛根性撕脱患者作为实践对象,利用正常人群与全臂丛撕脱患者作对照。患者在2016年12月—2018年2月入院就诊或者检查。利用fMRI对患者进行检查,将结果进行对比。结果 L-f MRI敏感度为50%(χ~2=1.01,P>0.05)。R-f NRI敏感度为60%(χ~2=3.42,P<0.05)。L-fMRI特异性为72.7%(χ2=4.12,P<0.05)。R-fMRI特异性为90.9%(χ~2=1.02,P>0.05)。B组、正常人群扫描结果显示,B组患者右手测运动区域连接系数为(0.254±0.061),指标明显低于左侧系数。并且B组患者整体系数低于正常人群,差异有统计学意义(P<0.05)。结论 fMRI及DTI对脑胶质瘤手术患者语言皮质功能检测敏感度不足,但是能够对全臂丛根性撕脱患者脑皮质重塑过程进行监测。
Objective To investigate the application of resting f MRI in brain function cortex localization and brain function remodeling. Methods 18 patients with primary glomerular glioma surgery and 22 patients with total brachial plexus avulsion were convenient selected and enrolled in our hospital. The normal population and the patients with total brachial plexus avulsion were used as control group. The patient was admitted to the hospital from December 2016 to February 2018. Patients were examined using f MRI and the results were compared. Results The L-fMRI sensitivity was 50%(χ~2=1.01, P >0.05). The R-fNRI sensitivity was 60%(χ~2=3.42, P<0.05). The specificity of L-fMRI was 72.7%(χ~2=4.12, P<0.05). The specificity of R-fMRI was 90.9%(χ~2=1.02, P>0.05). The results of group B and normal population showed that the connection coefficient of right-hand exercise area in group B was(0.254±0.061), and the index was significantly lower than the left side coefficient. And the overall coefficient of patients in group B was lower than that in normal population, and the difference was more obvious(P<0.05). Conclusion fMRI and DTI are not sensitive enough to detect cortical function in patients with glioma surgery, but can monitor the process of cortical remodeling in patients with total brachial root avulsion.
