巩膜隧道切口超声乳化和非超声乳化白内障摘除人工晶体植入后角膜地形图观察

Corneal topographic analysis of sclera tunnel small incision phacomusification or non-phaconoemusification in cataract surgery

目的观察巩膜隧道切口超声乳化和非超声乳化白内障摘除术联合人工晶体植入术对角膜形态的影响。方法两组老年白内障由同一人施行手术 ,非超声乳化组 4 2眼 ,超声乳化组 6 6眼 ,均采用 5～ 6mm巩膜隧道切口 ,用Zeis公司Humphrey计算机辅助的角膜地形图检查术后 1周、2周、1月、3月的角膜形态。结果术后 1周、2周的视力两组比较有显著性差异 (t=5 .5 787,3.935 4P <0 .0 1) ,1月、3月视力恢复状况无差别 (t=0 .6 930 ,0 .32 2 8,P >0 .0 5 ) ,两组术后散光状况亦提示短期有一定差别 ,phaco组在术后 1周、2周优于相应时期的N -phaco组 (t=5 .5 787,3.935 4P <0 .0 1)。CIM显示 1周、2周时两组不同 (t=4 .80 15 ,3.2 2 78P <0 .0 1) ,晚期角膜形态的恢复状况接近。SF的变化两组无明显区别 (P >0 .0 5 )。

Objective To study corneal topographic changes of both cataract surgery undergone phacoemusification or non-phaconoemusification through sclera tunnel small incision. Method 108 eyes(96cases)all were senile cataract ,divided into two groups phacoemulsification 66eyes (56cases) and non-phacoemusification 42eyes(40cases) was analyzed at 1week?2week?1month and 3month postoperatively. Result The center vision acuity compared with both groups at 1week?2week postoperatively was a statistical significantly ( t =2.5179, P <0.05),and was not difference at 2 week 1month,3month( t =1.7984,0.6930,0.3288, P >0.05) postoperatively.Surgically induced astigmatic changes in phacoemusification was lower than another groups after 1week,( t =5.5787,3.9354, P <0.01).CIM showed the difference at 1week,2week only( t =4.8015,3.2278, P <0.01).The change of SF was no obvious in various stagese. conclusion the long-term corneal topographic changes of both cataract surgery,through sclera tunnel small incision phacomusification or non-phacomusification was no obvious difference.
