经内镜十二指肠乳头括约肌切开术并发症的防治

THE METHODS TO PREVENT AND MANAGE COMPLICATIONS OF ENDOSCOPIC SPHINCTEROTOMY

目的 :探讨经内镜十二指肠乳头括约肌切开术 (endoscopicsphincterotomy ,EST)并发症的防治。方法 :应用OlympusTJF - 2 4 0型及FujinonED - 2 0 0XU型电子十二指肠镜、黄斑马导引丝、聪明刀等对 116例患者进行ERCP/EST治疗 ,其中并发症有胰腺炎 (8/116 ) ,胆管炎 (2 /116 ) ,出血 (3/116 )皮下气肿 (1/116 )等 ,对发生的并发症进行病因分析及治疗。结果 :回顾 116例EST的体会 ,结合文献 ,总结EST并发症的防治措施。结论 :通过提高EST的综合操作水平 ,并根据不同的病情选择治疗方案 ,可有效地预防和减少EST的并发症。

Objective: To discover methods to prevent and manage complications of endoscopic sphincterotomy (EST). Methods: Using electronic duodenoscope (Olympus TJF-240, Fujinon ED-200XU ) ,clever cut etc to treat 116 cases. Complications are the following: pancreatitis (8/116), cholangitis (2/116), hemorrhage (3/116), and pneumoderma (1/116). To analysis the causes of these complications and manage them. Results: Accumulating the phenomena of the 116 cases to discover methods to prevent and to manage complications of EST. Conclusions: Improving the skill at EST and using different managements for different cases can reduce the rate of these complications.
