摘要
目的 :探讨微血管计数 (MVC)对直肠癌远切段长度的影响 ,为手术时确定直肠癌远段切除长度提供参考指标。方法 :用Ⅷ因子相关抗原 (F8 RA)单克隆抗体SP免疫组化法测定 97例直肠癌组织的MVC ,并连续切片 ,在光镜下观测直肠癌的远端壁内扩散长度 ,并进行统计学分析。结果 :远端发生扩散的直肠癌 4 7例 ,平均MVC为 2 9 36± 5 14 ,明显高于无远端扩散 5 0例的MVC (18 2 4± 4 90 ) ,且扩散距离较长的直肠癌 ,MVC明显较高。结论 :MVC高的直肠癌 ,远端扩散距离较长 ,手术时远端一般切除 2 0～3 0cm即可。术前活检确诊直肠癌的同时测定MVC 。
Objective:To study the influence of microvessel count (MVCon distal clearance length of rectal cancer,and provide reference indexes for the distal clearance length.Methods:MVC was determined by immunohistochemical method in 97 patients with rectal cancer,the distal spread length was measured under light microscope.Results:In 47 patients with distal spread,the MVC (29 36±5 14) was significantly higher than that of 50 patients without distal spread (18 24±4 90).Conclusion:The MVC of preoperative bioptic specimens will help to choose the length of distal clearance.
