后路脊柱内窥镜下髓核摘除术治疗椎间盘钙化的腰椎间盘突出症 被引量:2

摘要 目的 探讨后路脊柱内窥镜下髓核摘除术 (MED)治疗椎间盘钙化的腰椎间盘突出症的手术方法及其效果。方法 用MED治疗椎间盘钙化的腰椎间盘突出症 67例 ,术中摘除髓核并同时扩大侧隐窝和神经根管。结果 术后随访5 0例 ,平均随访时间 1年 3个月。参照Nakano标准评定手术效果 ,优 :3 7例 ;良 :9例 ;可 :4例。结论 MED治疗椎间盘钙化的腰椎间盘突出症 ,创口小 ,出血量少 ,手术时间短。只要掌握好手术适应证和熟练的手术技巧 ,便能达到满意的效果。 Objective To investigate the operative procedures and its efficacy of the calcified lumbar disc herniations using microendoscopic discectomy(MED).Methods 67 cases of calcified lumbar disc herniations were performed by MED.The nucleus pulposus was extirpatied,The lateral recess stenosis and the nerve root canal were enlarged.Results With an average of 15 months follow up,50 patients were evaluated using Nakano's criteria.The results were classified as excellent in 37 cases,good in 9 cases and poor in 4 cases.Conclusions MED is a minimal invasive surgical procedure for the calcified lumbar disc herniation.Its small incision can decrease operative blood loss and reduce postoperative pain.Experienced operative skills of MED are the keys to successful treatment.
