手术和内窥镜治疗高龄急性化脓性胆管炎的比较

Comparison of Emergent Therapy of Traditional Operation on Endoscopic Manupilation for Acute Suppurative Cholangitis in Aged Patients

目的比较急诊手术和内镜治疗高龄急性化脓性胆管炎患者的疗效。 方法分析1995年1月-2001年10月,212例高龄(≥70岁)由结石引起的急性化脓性胆管炎患者手术和内镜治疗的疗效和合并症。 结果内镜组引流成功率100％,取石成功率达到88.5％,而且治疗后并发症发生率(3.8％)明显低于手术组(33.6％,P<0.01)。 结论内镜治疗高龄急性化脓性胆管炎的疗效与手术相仿,但安全性明显高于后者。

Objective To evaluate the effects of emergent traditional operation and endoscopic therapy for acute suppurative cholangitis ( ASC ) of aged patient. Methods A retrospective analysis of complications and healing of 212 ASC patients ( ≥70 yrs) from Jan. 1995 to Oct. 2001 was studied. Results Patients effective drainage ( 100% ) and bile duct stones were successfully removed in 88. 5 percent of patients undergoing endoscopic therapy, and its incidence rate of complications (3.8%) was significantly lower than that of traditional operation (33.6% , P<0. 01). Conclusion Endoscopic therapy for ASC can get the similar result of traditinal operation and more safety.
