摘要
目的 研究ras、P5 3基因突变和蛋白表达以及DNA倍体异常与尿路上皮肿瘤预后的关系。方法 用聚合酶链式反应加单链多态性分析 (PCR -SSCP)及流式细胞术和细胞免疫荧光技术检测 5 2例尿路上皮肿瘤K -ras和P5 3基因突变及蛋白表达和DNA含量变化。结果 K -ras、P5 3基因突变分别为 9.6 2 % ( 5 /5 2 )、42 .31% ( 2 2 /5 2 ) ;rasP2 1、P5 3蛋白阳性表达分别为 6 9.2 3 % ( 36 /5 2 )、6 3 .46 % ( 33/5 2 ) ;异倍体和二倍体肿瘤分别为 42 .31% ( 2 2 /5 2 )、5 7.6 9% ( 30 /5 2 ) ;DNA倍体异常、P5 3基因突变及rasP2 1和P5 3蛋白表达随肿瘤病理分级和分期的上升而增加 ,与肿瘤预后有直接关系(P <0 .0 1) ;异倍体肿瘤其P5 3基因突变、rasP2 1和P5 3蛋白表达阳性率明显高于二倍体肿瘤 (P <0 .0 1)。结论 DNA倍体异常、P5 3基因及rasP2 1和P5 3蛋白表达对尿路上皮肿瘤预后有预测意义。
Objective To research for the relationship between ras, P53 gene mutation, protein expression, abnormal DNA ploidy and the prognosis of urothelioma.Methods A PCR-SSCP technique and a flow cytometry technique and a cellular immunofluorescence technique were used to determine the K-ras, P53 gene mutation and the changes of DNA content in 52 patients with urothelioma.Results The positive rates of K-ras and P53 gene mutation were 9.62% (5/52) and 42.31% (22/52), respectively; the positive expression rates of rasP21 and P53 protein were 69.23% (36/52) and 63.46% (33/52), respectively; the aneuploidy and diploidy neoplasms were 42.31% (22/52) and 57.69% (30/52), respectively. The increasing of pathologic grading and staging of tumor resulted in the raising of the levels of abnormal DNA ploidy, P53 gene mutation and ras P21 and P53 protein expression, which were directly associated with the prognosis of neoplasms (P<0.01). The positive expression rates of P53 gene mutation, ras P21 and P53 protein in aneuploidy neoplasm were significantly higher than those in diploidy neoplasm (P<0.01).Conclusions Abnormal DNA ploidy, P53 gene, rasP21 and P53 protein expressions are of significance in forecasting the prognosis of urothelioma.
