90例中枢神经系统白血病的诊断与治疗

DIAGNOSES AND TREATMENT OF 90 CASES OF LEUKEMIA IN CENTRAL NERVOUS SYSTEM

目的 探讨中枢神经系统白血病 (CNSL)的诊断方法与治疗效果。方法 总结 1986年 5月至 2 0 0 1年 6月收治的 90例CNSL患者的诊断与治疗经验。结果 CNSL可发生于任何年龄 ,但青壮年高于老年人 ,主要症状表现为颅内压增高及颅神经损害。诊断主要依靠临床表现、体征及脑脊液 (CSF)检查 ,CSF涂片可找到幼稚细胞 ,治疗主要以鞘内注射化疗和颅脑脊髓照射为主。结论 提高CNSL诊断准确率 ,鞘内注射以采用脑室 腰部灌注给药法为优 ,结合颅脑脊髓照射可明显提高疗效。

Objective To explore the diagnoses and treatment of leukemia in central nervous system.Methods The diagnoses and treatment experience of 90 cases of leukemia in central nervous system treated from May,1986 to June,2001 were retrospectively reviewed here.Results Leukemia in certral system can be found out in people of any age,especially among young people and adults.Intracranial hypertension and injury of cranial nerve were the two main symptoms of the disease.The disease could be discovered by observing its clinical manifestations,body signs and by CSF detection.Juvenile cells could be discovered by observing CSF smears.Intrathecal injection and craniocerebral spinal irradiation were the two main treatment measures.Conclusion Accurate diagnosis,cerebral ventricular and lumbar perfusion and craniocerebral spinal irradiation can greatly improve the curative effects.
