Analysis of Hepatic Encephalopathy After TIPSS

目的 探讨TIPSS术后并发肝性脑病的原因和特点。方法 对 3 2例TIPSS手术患者临床资料进行回顾性分析。结果 2 8 1%的患者术后出现肝性脑病。肝性脑病的发生与术后门静脉血液流向有关 (P <0 0 1) ;与患者术前肝功能及手术前后门静脉压力的改变无明显关系 (P >0 0 5 )。所有的患者发生肝性脑病都存在诱因。

Objective To investigate the causes and characterization of hepatic encephalopathy after transjugular intrahepatic portosystemic shunts(TIPSS).Methods 32 cases of patients with recepted TIPSS operation were enrolled in retrospective study.Results 9 of 32 patients suffered from hepatic encephalopathy were detected out after TIPSS. The present of hepatic encephalopathy was associated with the flow direction of blood in portvein after the operation(P<0 01), but not with liver function before TIPSS(P<0 05) and the change of the tension of portvein. All the hepatic encephalopathy were induced by something.Conclusion Main high risk factor causing hepatic encephalopathy is presenting hepatofugal direction in partal vein blood flow.
