参麦注射液改善老年患者氧代谢的临床研究

Clinical study on the Shenmai injection(参麦注射液) improving the oxygen metabolism of old patients

目的 :探讨参麦注射液预防老年患者发生多器官功能障碍综合征 (MODS)的作用机制。方法 :将 14 6例老年患者随机分为两组 :常规病因治疗加参麦注射液 (参麦组 ) 73例 ,常规病因治疗 (对照组 ) 73例 ,共治疗7d。治疗前后测定两组患者的心排血量 (CO)、动静脉血气及动脉血乳酸值 ,并计算心脏指数 (CI)、氧供 (DO2 )和氧耗 (VO2 )。结果 :参麦组 DO2 和 VO2 均比对照组明显增高 (P<0 .0 1,P<0 .0 5 ) ,动脉血乳酸值比对照组显著下降 (P<0 .0 1) ;追踪观察参麦组 MODS发生率 (2 1.9% )比对照组 (47.9% )明显下降 (P<0 .0 1)。结论 :参麦注射液能改善老年患者“氧缺陷性氧耗”现象 ,提高氧供和组织摄氧能力 ,纠正酸中毒 ,防治老年 MODS。

Objective:To investigate the effect of Shenmai injection(参麦注射液) on preventing multiple organ dysfunction syndrome (MODS) of old patients.Methods:One hundred and fortysix old patients were randomly divided into Shenmai injection group and control group,73 patients in each group.Cardiac output(CO),arteriovenous blood gas and arterial blood lactate were monitored before and after therapy.Cardiac index(CI), oxygen delivery(DO 2) and oxygen consumption(VO 2) were calculated in both group.Results:DO 2 and VO 2 of Shenmai injection group were significantly higher than that of control group ( P <0 01, P <0 05),arterial blood lactate of Shenmai injection group was significantly lower than that of control group( P <0 01).The incidence of MODS on Shenmai injection group(21 9%) was significantly lower than that of control group (47 9%), P <0 01.Conclusion:Shenmai injection can improve the phenomena of defective oxygen consumption of old patients,increase DO 2 and extraction of oxygen of tissue,correct acidosis,and it is effective drugs to prevent MODS of old patients.
