复方丹参注射液对离体猪肾缺血再灌注损伤的作用研究 被引量:6

摘要 目的 :探讨复方丹参注射液 (co -SMI)减轻缺血再灌注 (IR)肾损伤的作用及机制。方法 :采用离体猪肾自体全血再灌注损伤模型 ,将 31只猪肾随机分为对照组 (n =11)、IR组 (n =11)和co -SMI组 (n =9)。供肾猪均电击制动。对照组开腹后 ,即取肾下极作检测 ;IR组和co -SMI组分别取再灌注 2h尿及再灌注 2 .5h肾组织作检测。结果 :IR组同对照组比较 ,肾组织中MDA、TNF -α、IL - 8含量较高 ,NO含量和SOD活力较低 ,差异均具有显著性 (P <0 .0 5 ,以下同 )。co -SMI组同对照组比较 ,肾组织中NO含量较低 ,TNF -α含量较高 ,差异均具有显著性 ;co-SMI组同IR组比较 ,MDA、IL - 8含量和尿蛋白含量较低 ,NO含量和SOD活力较高 ,再灌注 0 .5h至 2 .5h期间 ,灌注阻力减小 ,差异均具有显著性。co -SMI组光、电镜下肾组织结构损伤较IR组轻。结论 :co -SMI具有减轻离体猪肾IR损伤的作用 ,其机制与减少OFR蓄积、增加IR肾组织中NO含量以及降低IL - Objective: The purpose of the study was to observe the protective effects and the mechanisms of co-salvia milterrhizae injection(co-SMI) on the isolated swine kidney with ischemia reperfusion (IR).Methods: Thirty one swines were divided randomly into control group (n=11), IR group (n=11) and co-SMI treatment group (co-SMI group, n=9) in the study. All the swines were electrified,their kidneys removed and put in ice-box immediately for detecting in control group; in IR group and co-SMI group, reperfusion pressure and flow rate were both observed once every 30 minutes; the urine at the time of reperfusion for 2 hours and the renal tissue at the time of reperfusion for 2 hours and 30 minutes were taken for detecting. Results: The resistance of perfusion in co-SMI group was found much lower than that in the IR group during reperfusion except at the start of reperfusion, there was significant difference between IR group and co-SMI group(P<0.05, same for the following). The content of MDA and TNF-α and IL-8 in the renal tissue in IR group was higher, and the content of NO and activity of SOD was lower than that in control group. In co-SMI group, the content of NO in renal tissue was lower than that in control group , the content of TNF-was higher than that in control group. Compared with IR group, the content of MDA and IL-8 in the renal tissue and urine protein was lower and the activity of SOD and the content of NO was higher in co-SMI group. The serious degeneration and necrosis of renal tissue were observed in IR group under light microscope and electron microscope , but there was only slight degeneration in co-SMI group.Conclusion: The results indicated that co-SMI is effective in protecting the isolated swine kidney from IR injury. Its mechanisms might be: decreasing the content of OFR, increasing the content of NO, and inhibiting the increase of level of IL-8 in renal tissue.
