血管生成因子VEGF和bFGF与子宫内膜异位症的相关性研究

EXPRESSIONOF VASCULAR ENDOTHELIAL GROWTH FACTOR AND BASIC FIBROBLAST GROWTH FACTOR IN EUTOPIC AND ECTOPIC ENDOMETRIUM

目的 :检测血管内皮生长因子 (VEGF)和碱性成纤维细胞生长因子 (bFGF)在子宫内膜异位症 (EM)中的表达 ,探讨两种因子与EM发病机制的关系。方法 :收集EM患者的子宫内膜异位病灶组织 35例 ,在位内膜组织38例及非子宫内膜异位症患者的子宫内膜 37例 ,采用免疫组织化学SP法分别测定VEGF及bFGF在内膜组织中的表达情况 ,并用组织学评分对实验结果进行半定量统计 ,比较其表达强度。结果 :①VEGF在三组内膜的腺体及间质细胞中均有表达 ,主要定位于细胞浆。异位内膜组的表达强度高于正常对照组 ,二者差异有显著性 (P <0 .0 5 )。在位内膜组高于正常对照组 ,差异有显著性 (P <0 .0 5 )。异位内膜组同在位内膜组比较 ,差异无显著性 (P >0 .0 5 ) ;②EM患者的VEGF在异位内膜组和正常对照组均无周期性变化 ,而在EM患者的在位内膜中VEGF的表达强度分泌期比增生期明显增强 ,在月经周期中呈周期性变化 ;③bFGF在正常子宫内膜及EM患者在位内膜和异位内膜的增生期和分泌期中均有表达 ,表达强度差异无显著性 (P >0 .0 5 ) ,无周期性变化 ;④VEGF和bFGF在EM患者增生期和分泌期的在位内膜和异位内膜中的表达呈正相关关系。结论 :VEGF和bFGF均为促进血管生成的重要细胞因子 ,在子宫内膜异位症的异位内膜及在位内膜均有表达 。

Objective:To investigate the relationship of vascular endothelial growth factor (VEGF),and basic fibroblast growth factor(bFGF) with endometriosis.Methods:Immunohistochemistry was used to examine the VEGF and bFGF protein expression in the specimens of 38 cases with ectopic endometrium,35 cases with eutopic endometrium and 37 cases with normal endometrium.Histological score was used to semi-quantify the results.Results: The expression of VEGF in ectopic and eutopic endometrium was significantly higher than that in normal endometrium(P<0.05).Compared with eutopic endometrium,the expression of VEGF in ectopic endometrium showed no significance(P>0.05). No cyclic variation was observed in ectopic and normal endometrium.However,VEGF content was found to be higher in eutopic endometrium during secretory phase than that during proliferative phase.bFGF-positive cell was observed in all the three groups of endometrium.There were no significant differences(P>0.05).Conclusions:The pathogenesis and activity of endometriosis possibly depend on internal angiogenesis.Thus,VEGF and bFGF may play an important role in the pathogenesis of endometriosis.
