糖尿病患者血浆蛋白C活性变化的研究

检测34例糖尿病患者血浆蛋白C(PC)活性,抗凝血酶Ⅲ(ATⅢ)抗原和活性及抗血管性假血友病因子(Von Willebrand因子,vWF)含量,对其中9例进行治疗前后动态观察。糖尿病患者PC活性和vWF含量均高于对照组,伴微血管病变者PC活性高于不伴者,ATⅢ抗原和活性增高见于后者,前者与对照组比较差异无显著意义。PC活性与vWF及血清胆固醇含量呈正相关(r=0.5579,0.6712),与ATⅢ活性呈负相关(r=-0.5394)。治疗后PC活性和vWF含量均降低,ATⅢ抗原和活性无明显变化。提示糖尿病患者血浆抗凝活性受损,并与微血管病变及病情轻重有关。

In this study the plasma levels of protein C (PC) activity, antithrombin Ⅲ (AT Ⅲ) antigen and activity and von willebrand factor (vWF) were determined in 34 diabetics. These parameters of 9 out of the patients were observed before and after treatment. The value of PC activity and vWF in the diabetics were higher than those in the controls (P<0. 01). PC activity in the patients with retinopathy was increased, compared with those without retinopahy (P<0. 05). The elevation of AT Ⅲ antigen and activity was observed in the diabetics without retinopathy vs. the controls (P<0. 01), but no statistical difference was found between those with retinopathy and the controls. PC activity appeared positively correlated with vWF or serum cholesterol and negatively correlated with AT Ⅲ activity (P<0. 01). After treatment the plasma levels of PC and vWF were reduced, compared with those before treatment (P<0. 01, P<0. 05), whereas AT Ⅲ antigen and activity showed no difference. These data indicate that there is abnormality of anticoagulant system in diabetics. This change is related to the microangiopathy and the degree of dysbolism of diabetics.
