摘要
目的 :观察肝胃不和型功能性消化不良 (FD)大鼠胃和十二指肠运动功能、血浆胃动素 (MOT)含量和胃肠壁 P物质 (SP)的表达 ,探讨肝胃不和型 FD的发病机制及中药情志舒治疗肝胃不和型 FD的可能机制。方法 :将 6 4只大鼠随机分为对照组、模型组、多潘立酮组及情志舒组 ;应力传感器记录大鼠胃和十二指肠移行性复合运动 (MMC) ;放免法测定大鼠血浆 MOT含量 ;PAP免疫组化法观察 SP在胃窦壁和十二指肠壁的表达情况。结果 :与模型组比较 ,情志舒组 MMC周期缩短 , 相延长 ,频率加快 ,幅度升高 ,MMC 相发生率增加 ,胃和十二指肠协调收缩率显著增加 ;MMC 相时血浆 MOT含量显著增加 ;胃肠壁 SP免疫阳性产物表达也明显增强 (P <0 .0 1,<0 .0 5 )。结论 :胃肠 MMC 相异常、胃肠协调运动障碍可能是肝胃不和型 FD的发病机制之一 ;肝胃不和型 FD大鼠血浆 MOT含量的降低和胃肠壁 SP表达的降低可能是肝胃不和型 FD发病的神经生物学基础 ;情志舒可能通过增加 MMC 相血浆 MOT的释放和胃肠壁 SP的表达 ,从而改善胃肠运动功能来治疗肝胃不和型 FD。
objective: To investigate the antroduodenal motility, MOT and SP expression changes in rats with functional dyspepsia (FD) (type of incoordination between the liver and stomach) and to illustrate the possible mechanism of Qingzhishus effect. Methods: All rats were divided into 4 groups at random. The rat models of incoordination between the liver and stomach in FD were induced by stimulating the tails of the rats. Rats were implanted with strain gauges in the gastric antrum and duodenum to record the antroduodenal MMC. The content of MOT in plasma was determined by radioimmunoassay. The expression of SP in gastric antrum and duodenum was investigated by immunohistochemical assay. Results: Compared with that in the model group, the period of MMC in the Qingzhishu group was shortened, the phase Ⅲ of MMC prolonged, with more frequency and higher amplitude, the percentage of antroduodenal coordinative contraction increased; the levels of plasma MOT in any other groups were higher, the expression levels of SP in the gastric antrum and duodenum in other groups were higher (P< 0.01 ,< 0.05 ). Conclusion: The possible mechanism of functional dyspepsia is the abnormity of phase of MMC and the failure of antroduodenal coordinative contraction, which may be due to the changes of MOT and SP. Qingzhishu can regulate them to some extent to cure the FD with incoordination between the liver and stomach.
