摘要
目的:观察颈椎各节段黄韧带突入椎管的距离与后伸角度的关系,探讨颈椎后伸过程中黄韧带突入椎管的动态变化.方法:离体正常C2~T1节段成人脊柱标本14具,去除椎管内脊髓及硬膜,在各节段黄韧带内壁处放置自行研制的电阻应变式位移传感器探针并将输出信号连接于自动应变仪,在逐渐增大颈椎后伸角度的过程中读取各节段黄韧带突入椎管距离的数据,经统计处理描记出各节段黄韧带突入椎管距离与颈椎后伸角度之间的关系曲线.结果:过伸位黄韧带突入椎管距离:C5-6>C4-5>C6-7>C3-4>C7-T1>C2-3;C2-3至C7-T1六个节段黄韧带突入椎管距离与颈椎后伸角度之间的关系曲线均为'S'形;曲线上升过程中斜率由小变大的一点所对应的角度约为15°~33°,随黄韧带节段下降角度逐渐增大.结论:过伸位黄韧带突入椎管距离以C5-6为最大,其上、下方节段突入距离逐渐变小;用各节段黄韧带突入椎管距离与颈椎后伸角度关系的'S'形曲线解释了黄韧带在颈椎后伸过程中形态的动态变化.
Objective:To obtain the bulging curve of ligamentum flavum(LF) bulge of different segments of cervical vertebra with backward extension angle of neck.To illustrate the dynamic bulging transformation of ligamentum flavum during backward extension of the neck.Method:14 cervical vertebra specimens(C2-T1) were collected.After get rid of the spinal cord and putamen,strain sensitivity probe were placed in the corresponding parts of the vertebra canal to detect the inburst degree of LF.The output data were shown and recorded in an automatic strain gauge.Through statistical methods,data were measured and curves of all segments of LF inburst degree during the course of backward extension were drawn.Result:In over-extension position,LF bulge degree:C5-6>C4-5>C6-7>C3-4>C7-T1>C2-3; Curves of 6 different segments (C2-3-C7-T1) of LF bulge degree with backward extension angle were obtained,it is a 'S' shape curve,the corresponding angle of slope commonly changed point of curve range from 15° to 33°,the angle got larger as the counting down of the LF segments.Conclusion:C5-6 has the largest LF bulge degree in over-extension position.The upper and lower segments count down gradually; The 'S' shape curves can be explained by the different process of LF bulge that contribute to the formation of the curves;The upper segments LF get bulge earlier than lower segments on our experimental specimens.
