尿道损伤163例的早期治疗

Immediate Management for 163 Cases with Urethral Injury

目的 探讨尿道损伤的早期治疗方法。方法 对 10 2例前尿道损伤分别采用留置导尿管、膀胱造瘘、尿道会师和尿道吻合术四种方法治疗 ;6 1例后尿道损伤分别采用尿道会师和膀胱造瘘两种方法治疗。结果 前尿道损伤轻者采用留置导尿管和膀胱造瘘处理均取得较满意效果 ;前尿道损伤重者采用尿道吻合术和尿道会师术均取得满意效果。后尿道损伤行尿道会师成功率达 91.8% ,而膀胱造瘘组全部发生尿道闭锁、二期手术。结论 前尿道损伤根据损伤程度采用不同方法治疗 ;尿道会师术是治疗后尿道损伤简便、有效。

Objective TO evaluate various methods for immediate treatment of urethral injury Methods 102 cases with bulbar urethral injury were treated with urethral catheterization,suprapubic cystostomy,urethral realignment and piymary urethral anastomosis 61 cases with posterior urethral disruption were treated by urethal realignment and suprapubic cystostomy Results Satisfactory results had been achieved in sliht or mediate bulbar urethral injury undergoing urethral catheterization and suprapubic cystostomy,and in severe urethral injnry undergoing urethral realignment and primary urethral anastomosis 91.8% of successful rate had been achieved in posterior urethral injury treated with urethral realignment,while urethral obliteration was achieved in all undergioing suprapubic cystostomy Conclusion Different methods are adopted for bulbar urethral injuy by injury severity Urethral realignment is simple,effective and safe method for posterior urethral disruption
