摘要
目的 :观察腹腔感染后肠粘膜结构和上皮细胞增生代谢的改变以及生长激素 (GH)对感染后肠粘膜的影响。 方法 :采用盲肠结扎穿孔 (CLP)和经颈静脉插管微量输液泵输注建立腹腔感染及静脉营养大鼠动物模型。研究分为正常对照组、TPN组、感染加TPN组和GH组、感染加TPN组。后三组经颈静脉插管微量输液泵给予静脉营养 ,GH组同时每天给予GH 1U/kg。检测小肠粘膜厚度、隐窝深度和绒毛高度 ;并观察小肠粘膜上皮增生细胞核抗原 (PCNA)表达和肠粘膜上皮细胞增生率变化。 结果 :TPN后小肠粘膜厚度、隐窝深度和绒毛高度呈萎缩趋势 ,肠上皮细胞增生率下降 ,感染加重。GH可减轻感染大鼠TPN后小肠粘膜萎缩 ,并促进肠上皮细胞增生和肠粘膜PCNA表达。 结论 :腹腔感染后小肠粘膜呈现明显萎缩变化 ,肠粘膜上皮细胞增生下降。GH可促进感染后小肠粘膜上皮细胞增生。
Objectives:To observe changes of intestinal mucosa morphology and proliferation after infection.Moreover, to observe influence of growth hormone on intestinal mucosa proliferation. Methods:Intra abdominal infection model was induced by cecal ligation and puncture and continuous intravenous nutrition was followed by transjugular intracaval catheterization.Experimental rats were divided into four groups:control,TPN control and TPN plus sepsis growth hormone,TPN plus sepsis groups.Changes of mucosa thickness,villus height,crypt depth,proliferating cell nuclear antigen(PCNA),epithelium cells proliferation index were determined. Results:Rat intestinal mucosa appears to have an obvious tendency to atrophy,proliferation of epithelium cells are markedly inhibited in TPN.During sepsis states,all above changes became worse.Growth hormone promotes growth of intestinal mucosal cell,improves proliferation of epithelium cells and expression of mucosa PCNA. Conclusions:During sepsis,intestinal mucosa appears to become atrophy,proliferation of mucosal epithelium cells markedly decreases.Growth hormone accelerates proliferation of intestinal mucosa epithelium cells and lightens mucosa atrophy.
