摘要
目的 探讨强直刺激与切皮引起循环及脑电图变化的相关性。方法 33例ASAⅠ~Ⅱ级择期神经外科手术病人 ,麻醉诱导后 ,以地氟醚吸入维持在 1 0MAC超过 10min ,于前臂尺侧经予 3倍耐痛阈的强直刺激。刺激 5min后切皮。分别记录刺激及切皮前后血压、心率及脑电双频谱指数 (BIS)的变化。结果 强直刺激及切皮后血压、心率均显著升高 (P <0 0 1) ,而BIS无明显变化(P >0 0 5 )。强直刺激与切皮前后平均动脉压和心率的增长幅度具显著相关性 (r分别为 0 94和0 75 ,P <0 0 1)。结论 强直刺激和切皮引起的血液动力学变化有明显的相关性。用强直刺激 循环反应来预测切皮
Objective To observe the difference of EEG changes and circulaory responses to tetanus stimulus(TS)and skin incision.Methods Thirty three patients (classified as ASAⅠ Ⅱ)scheduled for neurosurgery were included in this study.After induction,anesthesia was maintained with 1 0MAC desflurane inhalation for more than 10min.The mean arterial pressure(MAP),heart rate (HR) and EEG bispectral index(BIS)were recorded after TS(3times the maximal pain tolerance and lasting for 5s) applied to the ulnar nerve at the wrist and during skin incision,respectively.Results MAP and HR were increased significantly after TS or skin incision( P< 0 01),but BIS did not changed significantly( P> 0 05).There was a significant possitive correlation( r= 0 94 for MAP and 0 75 for HR, P< 0 01),in the increase in amplitude of MAP and HR between after TS and during skin incision.Conclusion There is a significant correlation in the hemodynamic changes between after TS and during skin incision.It is possible to predict the incision circulatory responses with TS circulatory responses.
