全麻控制性降压对小儿鼻内窥镜手术中出血的影响

Effect of deliberate hypotension on the bleeding of pediatric endoscopic endonasal surgery under general anesthesia

目的 :为减少小儿鼻内窥镜手术中的出血 ,我院采用全麻控制性降压 ,从而达到顺利完成手术的目的。方法 :将72例小儿鼻息肉、鼻窦炎患者随机分成A、B两组 ,A组实行单纯全麻下手术 ,B组实行全麻控制性降压 ,术中均用2 %利多卡因20ml加1‰肾上腺素4ml浸润的棉片止血 ,观察A、B两组术中出血情况。结果 :实行全麻控制性降压的B组比单纯全麻下手术A组术中出血平均减少76ml,差异有显著性(P<0.001)。结论

Objective:In order to reduce the bleeding in pediatric endoscopic endonasal surgery,We adopted the deliberate hypotension under general anesthesia for this operation.Methods:72 patients with pediatric nasal polyp and sinusitis were randomly divided into group A and group B.The operation in the group A was done,of under the condition mere general anesthesia,While the operation in the group B with deliberate hypotension under general anesthesia.2% lidocaine 20ml plus 1‰ adrenalin 4ml was used in both groups for prevent bleeding.The condition of bleeding,in two groups was observed. Results:The average bleeding amount of group B (90ml) was less than that of group A (P<0.001). Conclusion:Deliberate hypotension in general anesthesia can reduce significantly the bleeding in pediatric endoscopic endonasal surgery.
