A study of consistency of occlusal type with skeletal pattern and integumental profile

目的 :深入了解型、骨型、软组织面型在各类错中的相互关系 ,探讨错形成机制。方法 :随机抽取12～ 15岁安氏各类错患者 4 4 0名及正常者 6 0名 ,采用概率计算法得出各类错骨型与软组织面型的分布 ,并计算各类错之间以及它们与正常间 13个颅面骨骼基本组分变异范围的交叉范围。结果 :型与骨型、型与软组织面型之间不一致的现象以Ⅱ类错最为明显 ,其次是Ⅲ类错 ,Ⅰ类错差别最小 ;各类错以及它们与正常间颅面骨骼基本组分的变异范围存在相当大的交叉 ,彼此之间没有明确的分界点。结论 :为做出合理的矫治设计 ,达到更好的矫治效果 ,有必要对型、骨型、软组织面型鉴别诊断 ;大部分错不存在明显的骨面型异常 ,骨面型异常主要是代偿机制失调所致 ,垂直向的代偿能力比矢状向强。

Objective: To understand the consistency of occlusal type with skeletal pattern and integumental profile in each malocclusion group extensively, and to make an inquiry into the mechanism of malocclusion. Methods: 440 patients of three Angle's malocclusions and 60 normal subjects aged from 12-15 years were chosen randomly. The distributions of probability of skeletal pattern and soft tissue facial type were calculated in each malocclusion group contrasted to normal group, the percentages of common ranges between each pair of groups among Angle's three malocclusions and normal group were calculated too. Results: The inconsistency of occlusal type with skeletal pattern and integumental profile was most common in class Ⅱ malocclusion, least common in class ,and class Ⅲ was in between; There were no clear boundaries between any pairs of groups, and common ranges were quite wide. Conclusion: To give reasonable diagnosis and treatment plan, and to get best results from treatment, it's necessary to differentiate occlusal type, skeletal pattern and soft tissue facial pattern. Majority of malocclusion patients have no obvious anomaly on skeletal pattern, which results from ill match of skeletal components, and the ability of compensating is stronger vertical than sagittal.
