自拟化瘀通腑汤治疗急性硬膜外血肿49例疗效观察

The Observation on the Curative Efficacy of Huyu Tongfu Decoction in Treatment for Acute Extradural Hematoma

目的 探讨中西医结合治疗急性硬膜外血肿疗效及机理。方法 89例急性硬膜外血肿患者随机分为自拟化瘀通腑汤治疗组和对照组。自拟化瘀通腑汤治疗 4 9例 ,西医治疗组 4 0例 ,两组均用 2 0 %甘露醇及神经细胞活性药物作基础治疗 ,自拟化瘀通腑汤治疗组在西医常规治疗基础上 ,口服化瘀通腑汤治疗。结果 治疗组总有效率 91.8% ,血肿消失平均时间 2 4天 ;对照组总有效率 75 % ,两组比较差异有显著性 (P <0 .0 5〉 ,血肿消失平均时间 35 .4天 ,两组比较 ,差异有极显著性 (P <0 .0 1)。结论 化瘀通腑汤治疗急性硬膜外血肿不但优于单一西医治疗方法 ,而且可缩短病程 ,改善其症状 。

Objective To explore the curative efficacy and mechanism of integrated traditional Chinese and Western medicine (TCM-WM) on acute extradural hematoma (AEDH). Methods Eighty nine patients of AEDH were allocated randomly into the TCM-WM treatment group and control group. Both treatment group (n=49) and control group (n=40) were tread with 20% mannitol 250 ml and dexamethasone 5～10 mg infused intravenously twice a day and active drugs of neural cell as basic therapy. Based on those therapy Huyu Tongfu Decoction given orally was added. The therapeutic course was 7 days for treatment group. The two groups were compared and analyzed. Results Total effective rate was 91.8%, average time for the absorption of AEDH was 24 days in treatment group, but 75% and 35.4 days respective in control group. There was a significantly different between two groups (P<0.05 and P<0.01, respectively). Conclusions Huyu Tongfu Decoction possesses significantly curative efficacy better than that of only with Western medicine, shorts therapeutic course, promote the absorption of hematoma and increase the cured rate.
