CT导向下经皮穿刺术的临床应用价值

The clinical application value of percutaneous puncture in CT guiding

目的 探讨CT导向下经皮穿刺术的临床应用价值。方法 收集经CT导向下经皮穿刺术 71例 ,其中颅内血肿微创穿刺定位 34例 ,胸腹肿瘤细针穿刺活检定位 17例 ,腰椎间盘突出胶原酶溶解穿刺定位 8例 ,腹腔肝、肾、盆腔囊肿及脓肿穿刺抽吸定位 7例 ,肝癌射频消融穿刺治疗定位 4例 ,对定位穿刺方法、过程及注意事项进行了详细介绍。结果 穿刺病灶成功率为 10 0 % ,均在 1- 3针内成功 ;穿刺后出现的并发症总计为 2 2 .9% ,以颅内血肿微创的并发症发生率较高 ,为 32 .4 % ,且较严重 ,须加强注意观察 ,其余各项穿刺总并发症发生率为 13.9% ,不需特殊处理。结论 CT导向下经皮穿刺术安全简便、定位准确、创伤性小 ,可广泛地应用于临床 。

Objectives To Study the clinical application value of percutaneous puncture in CT guiding.Methods 70 cases with percutaneous puncture in CT guiding were collected,includeing 34 cases with puncture and localization of slight wound for intracranial hematoma,17 cases with needle puncture biopsy localization for chest and abdomen tumours,8 cases with puncture and localization of collagenase dissolve for prolapse of lumbar intervertebral disc, 7 cases with puncture and suction localization for cyst and abscess of abdomen and pelvic cavity, 4 cases with localization of radio frequency ablation for hepatocarcinoma. The method, process and announcements were detailedly introduced. Results The rate of the success of puncture were 100 percent, all at 1-3 needles. The complications had been up to 22.9% with puncture, with the occurrence rate of complications of slight wound in cranium being higher and more serious(32.4%),so observation should enhance. the rest were lower(13.9%), not to need special settlement.Conclusions Percutaneous puncture with CT guiding could been widely applied to clinic and full of vast prospect for safety,simple and convenient,localization accurately and slight wound.
