左布比卡因局部浸润麻醉对腹腔镜胆囊切除术后超前镇痛的研究

Research on preemptive analyesia by local infiltration anaesthesia with levobupivacaine in laparoscopic cholecysteetomy

目的探讨切口局部注射左布比卡因对腹腔镜胆囊切除术(LC)超前镇痛的作用。方法病人随机分为二组,每组20例,A组(n=20)患者手术结束前以0.5%的左布比卡因在切口皮下、肌层、腹膜内浸润,B组(n=20)患者不作特殊处理。各时段镇痛效果采用10 cm视觉模拟评分法(VAS)间接评估。结果术后2～8 h时段A组病人VAS评分明显小于B组(P<0.05),24 h时点两组VAS评分无显著性差异。结论手术结束前切口局部注射左布比因对LC术后具有良好的镇痛效果。

Objective:To discuss the effects of preemptive analgesia by local infiltration anesthesia with lev-obupivacaine in laparoscopic cholecystectomy.Methods: 40 patients were divided into two groups randomly,group A(n=20) received infiltration anesthesia with 0.5% levobupivacaine in the subcutaneous tissue,the muscular layer and the peritoneum beneath the incision before the end of the operation,group B(n=20) re-ceived routine treatment,then assessed the degree of the pain by using 10 cm visual analog scale method.Results: In the early postoperatively time(2,4 and 8 h),the VAS scores of A group were lower than B group obviously(P < 0.05).There was no significant difference between the two groups at 48 h.Conclu-sions: The analgesic effect with levobupivacaine before the end of the LC was better than postoperative analgesia.
