宫血宁联合屈螺酮炔雌醇片治疗青春期功能失调性子宫出血临床观察

Clinical research of Gongxuening capsule combined with drospirenone-ethinylestradiol tablets on adolescent dysfunctional uterine bleeding

目的探讨宫血宁联合屈螺酮炔雌醇片治疗青春期功能失调性子宫出血的效果。方法选择揭西县人民医院2015年1月至2016年3月收治的97例青春期功能失调性子宫出血患者,按随机数字表法分为两组,对照组(49例)给予屈螺酮炔雌醇片,治疗组(48例)给予宫血宁胶囊联合屈螺酮炔雌醇片,两组均治疗3个月,比较两组治疗效果。结果两组均有46例完成治疗,治疗组出血控制时间和完全止血时间[(21.4±5.6)h、(33.5±7.9)h]均少于对照组[(28.1±6.3)h、(46.2±8.2)h];治疗后治疗组子宫内膜厚度低于对照组[(4.6±1.5)mm vs(6.8±1.9)mm],血红蛋白含量[(128.5±14.2)g/L]高于对照组[(117.8±13.6)g/L],凝血酶原时间(prothrombin time,PT)和部分凝血活酶时间(activated partial thromboplastin time,APTT)[(9.4±1.1)s、(21.7±2.0)s]少于对照组[(12.6±1.7)s、(29.6±2.7)s],WHOQOL-BREF量表生理领域、心理领域、社会关系领域、环境领域评分均高于对照组,治疗总有效率高于对照组,复发率低于对照组,差异均有统计学意义(P<0.05)。结论宫血宁胶囊联合屈螺酮炔雌醇片治疗青春期功血可有效快速止血,提高疗效,降低复发率,值得临床推广。

Objective To investigate the clinical curative effect of Gongxuening capsule combined with drospirenone-ethinylestradiol tablets on adolescent dysfunctional uterine bleeding. Methods 97 patients with adolescent dysfunctional uterine bleeding admitted to People’s Hospital of Jiexi County from January 2015 to March 2016 were randomly divided into 2 groups.The control group(49 cases) were given drospirenone-ethinylestradiol tablets,and the treatment group(48 cases)were given Gongxuening capsule combined with drospirenone-ethinylestradiol tablets. Both groups were treated for 3 months, and the effects of the two groups were compared. Results 46 patients in both groups were treated. The bleeding control time and complete hemostasis time in the treatment group [(21.4±5.6)h,(33.5±7.9)h] were less than the control group ((28.1±6.3)h,(46.2±8.2)h)The endometrial thickness of the treatment group was lower than that of the control group [(4.6±1.5)mm vs(6.8±1.9)mm], and the Hb content(128.5±14.2) g/L was higher than that of the control group(117.8±13.6) g/L, prothrombin time(PT) and activated partial thromboplastin time(APTT) [(9.4 ± 1.1) s,(21.7 ± 2.0) s] were less than the control group [(12.6 ± 1.7) s,(29.6 ± 2.7) s].The WHOQOL BREF-scale physiological, psychological, social relations, environmental score were higher than those of the control group, the total effective rate was higher than that of the control group, and the recurrence rate lower than that of the control group, the differences were statistically significant(P<0.05). Conclusion Gongxuening capsule combined with drospirenone-ethinylestradiol tablets in treatment of adolescent dysfunctional uterine bleeding can effectively stop bleeding, improve the curative effect and reduce the recurrence rate, which is worthy of clinical promotion.
