产后盆底功能障碍行电刺激生物反馈、阴道锥联合盆底肌锻炼治疗的临床效果研究 被引量:57

Clinical effect of postoperative pelvic floor dysfunction with electrical stimulation biofeedback and vaginal cone combined with pelvic floor muscle exercise

摘要 目的探讨产后盆底功能障碍行电刺激生物反馈、阴道锥联合盆底肌锻炼治疗的临床效果。方法选取2015年6月至2017年6月于惠州市第六人民医院进行治疗的盆底功能障碍性疾病患者110例,依据随机数字表法分为观察组和对照组,各55例。对照组行盆底肌肉锻炼联合阴道锥训练治疗方案,观察组在对照组基础上联合电刺激生物反馈治疗方案。比较两组患者临床症状、盆底肌纤维肌力以及盆底肌纤维平均电位变化。结果治疗前对两组患者进行盆底功能障碍问卷(pelvic floor distress inventory-short form-20,PFDI-20)调查,差异无统计学意义(P>0.05)。治疗后两组患者PFDI-20评分明显改善,观察组优于对照组,差异均有统计学意义(P<0.05)。治疗后观察组患者盆底肌纤维肌力正常率优于对照组,差异有统计学意义(P<0.05)。两组患者盆底肌纤维平均电位治疗前无明显差异(P>0.05),治疗后均有明显改善;观察组优于对照组,差异均有统计学意义(P<0.05)。结论产后盆底功能障碍行电刺激生物反馈、阴道锥联合盆底肌锻炼治疗,能够改善盆底肌力及盆底肌电位,有效改善患者临床症状。 Objective To explore the clinical effects of electrical stimulation biofeedback and vaginal cone combined with pelvic floor muscle exercise on postoperative pelvic floor dysfunction. Methods A total of 110 patients with pelvic floor dysfunction disease who were treated in the Sixth People’s Hospital of Huizhou from June 2015 to June 2017 were enrolled. According to the random number table method, they were divided into observation group and control group, 55 cases each. The control group underwent pelvic floor muscle training combined with vaginal cone training treatment. The observation group was combined with electric stimulation biofeedback treatment on the basis of the control group. The clinical symptoms, pelvic floor muscle fiber strength and pelvic floor muscle fiber mean potential changes were compared between the two groups. Results Before the treatment, the pelvic floor distress inventory-short form 20(PFDI-20) was performed in the two groups. The difference was not statistically significant(P>0.05). After treatment, the PFDI-20 scores in the two groups were significantly improved(P<0.05), and the difference was statistically significant. After treatment, the PFDI-20 score improved significantly in the observation group.The difference was statistically significant(P<0.05). After treatment, the normal rate of pelvic floor muscle fiber strength was significantly better in the observation group than in the control group, the difference was statistically significant(P<0.05). There was no significant difference in the average pelvic floor muscle fiber between the two groups before treatment(P>0.05). After treatment, the average pelvic floor muscle fiber potential was significantly improved in the two groups(P<0.05). After treatment, the average potential of pelvic floor muscle fibers in the observation group was more significant, and the difference was statistically significant(P<0.05). Conclusion Postpartum pelvic floor dysfunction with electrical stimulation biofeedback, vaginal cone combined with pelvic floor muscle exercise treatment, can improve pelvic floor muscle strength and pelvic floor muscle potential, and effectively improve the clinical symptoms of patients.
