重症肌无力外科治疗72例报告

Surgical Therapy for Myasthenia Gravis(Report of 72 Cases)

目的 :探讨重症肌无力外科治疗的机理、疗效及围手术期处理 .方法 :胸骨正中入路或胸腺瘤切除及周围脂肪组织清扫 .结果 :术后死于呼衰 2例 ,术后 3a死于复发 1例 ,87%病例有效 (完全停药 +小剂量维持 ) ,13%无缓解 .结论 :重症肌无力病人 ,特别是有胸腺增生或胸腺瘤者 ,应积极手术治疗 。

Objective: To explore the pathogenesis, therapeutic effect and perioperative treatment for myasthenia gravis. Methods: Approach to sternum median or thymectomy and peripheral adipose tissue's sweeping. Results: After operation, 2 cases died for respiratory failure, 1 case for recurrence. The surgical therapy for 87% of all cases were effective (entirely stopping treating + minimal dose maintaining), but other 13% no relief. Conclusion: myasthenia gravis patients, especially those with thymus hyperplasia or thymoma, should actively take surgical therapy, and the majority of them can be cured.
