综合性评估与个体化干预对糖尿病慢性伤口患者焦虑及伤口愈合的影响

The effects of comprehensive evaluation and individualized intervention on anxiety and chronic wounds healing in patients with diabetes

目的:探讨综合性评估及个体化干预对糖尿病合并慢性伤口患者焦虑情绪及伤口愈合的影响。方法:将88例2型糖尿病合并慢性伤口患者随机分为干预组和对照组各44例。两组患者均采取湿性愈合,对照组接受常规治疗及护理措施;干预组在常规基础上给予综合性评估与个体化干预,比较两组患者焦虑评分及伤口愈合情况。结果:进行综合性评估与个体化干预后,干预组焦虑分值低于对照组,具有统计学差异(t=-4.178,P<0.05);干预组焦虑程度在干预4周(χ~2=12.302,P<0.05)和8周后(χ~2=5.643,P<0.05)均低于对照组,具有统计学差异;干预组伤口愈合面积达25%(t=-2.525,P<0.05)、50%(t=-3.952,P <0.05)、75%(t=-2.386,P <0.05)、100%(t=-3.349,P<0.05)所用时间均少于对照组,具有统计学差异。结论:对糖尿病合并慢性伤口患者实施综合性评估与个体化干预,可显著改善患者焦虑情绪,有效缩短伤口愈合时间。

Objective:To explore the effects of comprehensive evaluation and individualized intervention on anxietyand wound healing of patients with diabetes mellitus combined with chronic wound.Methods:A total of 88patientswith typeⅡdiabetes mellitus with chronic wounds were randomly divided into two groups:The intervention groupand the control group,each with 44patients,Both groups received wet healing method treatment and routine nursingmeasures,on this basis,the intervention group was given comprehensive evaluation and individualized intervention,The anxiety score and wound healing were compared between the two groups.Results:After comprehensive evaluation and individualized intervention,the anxiety score of the intervention group was lower than that of the controlgroup,the difference was statistically significant(t=-4.178,P<0.05),The level of anxious in the intervention groupwas statistically significant lower than that in the control group when the intervention measures had been given forfour weeks(χ2=12.302 P<0.05)and eight weeks(χ2=5.643 P<0.05).The wound healing time in the interventiongroup was less than that in the control group when the chronic wounds had healed 25percent(t=-2.525 P<0.05),50percent(t=-3.952 P<0.05),75percent(t=-2.386 P<0.05)and 100percent(t=-3.349 P<0.05).Conclusion:The comprehensive evaluation and the individualized intervention for diabetes mellitus combined with chronic woundcan significantly improve the patient's anxiety and shorten the wound healing time.
