治疗性沟通系统对凶险性前置胎盘伴胎盘植入产妇心理状态及康复效果

Effect of therapeutic communication system on mental status and recovery of pernicious placenta previa and accreta

目的:探讨治疗性沟通系统对凶险性前置胎盘伴胎盘植入产妇心理状态及康复效果的影响。方法:选取2011年12月至2017年12月我院收治的75例凶险性前置胎盘伴胎盘植入产妇作为研究对象,采用回顾性分析法分析所有产妇的临床资料,按照资料中其治疗方法将其分为对照组37例和研究组38例,对照组患者在所患疾病临床治疗上予以常规护理干预,研究组在对照组基础上予以治疗性沟通系统,两组均干预8次,干预结束后比较两组产妇其情绪、日常生活能力、生活质量以及康复治疗依从性的变化情况。结果:①两组产妇干预后其SAS、SDA评分均较干预前降低,Barthel指数评分均较治疗前升高,且研究组降低、升高幅度明显大于对照组,比较差异具有统计学意义(t=5.513,6.057,6.579;P<0.001);②两组产妇干预后生活质量评分较干预前明显升高,且研究组升高程度明显大于对照组,比较差异具有统计学意义(t=6.847,5.843,6.157,6.518,7.052,5.986;P<0.001);③研究组干预后康复治疗依从性(86.84%)明显高于对照组(64.86%),比较差异具有显著性(χ~2=12.035,P<0.001)。结论:对凶险性前置胎盘伴胎盘植入产妇实施治疗性沟通系统可明显减轻其焦虑和抑郁程度,提高日常生活能力、生活质量及康复治疗依从性,可作为凶险性前置胎盘伴胎盘植入治疗者首选的干预模式。

Objective:To investigate the effect of therapeutic communication system on mental status and recovery of pernicious placenta previa and accreta.Methods:The clinical data of 75 pernicious placenta previa and accreta treated in our hospital from December 2011 to December 2017 were retrospectively analyzed.All patients received symptomatic clinical treatment,based on this,37 patients in the control group received routine nursing intervention,and 38 patients in the study group were given the therapeutic communication system,both interventions were given for 8 times.The changes of mood,daily living ability,quality of life and rehabilitation treatment compliance were compared between the two groups.Results:①The SAS and SDA scores of two groups were decreased after intervention,and the Barthel index scores were increased,moreover,both increasing level and decreasing level were more obvious in the study group than those in the control group(t=5.513,6.057,6.579;P<0.001);②The quality of life score in two groups significantly increased,and the increasing level in the study group was more significant than that in the control group(t=6.847,5.843,6.157,6.518,7.052,5.986;P<0.001);③The study group’s rehabilitation treatment compliance(86.84%)was significantly higher than that of the control group(64.86%),the difference was significant(χ~2=12.035,P<0.001).Conclusion:The therapeutic communication system can obviously reduce the anxiety and depression,improve their daily living ability,quality of life and rehabilitation treatment compliance for pernicious placenta previa and accreta,which can be used as preferred mode of intervention.
