探讨血清胱抑素C对诊断2型糖尿病肾病的意义

Urinary inhibition to explore serum C on the significance of diagnosis of type 2 diabetic nephropathy

目的:探讨血清胱抑素C(CySTaTin C,CyS-C)对诊断2型糖尿病肾病的意义。方法:将127例2型糖尿病患者根据尿白蛋白排泄率(uaer)分为正常蛋白尿组(a组)、微量蛋白尿组(B组)和临床蛋白尿组(C组)3组,并选择同期的40例健康体检者作为对照组。分别采用两点速率法、固定时间法、终点法检测并分析四组患者血清胱抑素C(CyS-C)、尿素氮(Bun)、血肌酐(SCr)指标。结果:糖尿病组(a、B、C组)CyS-C水平显著高于对照组,各组间差异有统计学意义,且随着尿白蛋白排泄率(uaer)的增高,均有明显升高,CyS-C水平也随之增高;在微量蛋白尿组(B组)CyS-C、Bun、SCr的阳性率分别为58%、18.9%、12.7%,经X2检验,血清CyS-C检测阳性率与Bun、SCr比较差异有显著性(p<0.01)。结论:CyS-C可用于诊断糖尿病肾病,且在一定程度上反映2型糖尿病肾病患者肾损害的程度,CyS-C在2型糖尿病肾病早期有较高的阳性检出率,对诊断有重要意义。

Objective:To study the value of the serum cystatin C(Cystatin C,Cys-C)in patients with diabetic nephropathy in early diagnosis.Methods:127patients with type 2diabetes,according to Chung by urinary albumin excretion rate(UAER)were divided into 3groups:the normal albuminuria group(the group A),the microalbuminuria group(the group B)and the clinical proteinuria group(the group C),and select the same40 healthy subjects as the control group.Respectively by the methods of two rate,fixed time,end point detection and analysis of four groups of patients(Cys-C),urea nitrogen(BUN),serum creatinine(Scr)index.Result:Diabetes mellitus group(group A,B,C)Cys-C were significantly higher than that of control group,difference between groups was statistically significant,and with increased urinary albumin excretion rate(UAER)and has the obvious rise,Cys-C level is higher;In the trace albuminuria group(group B)Cys-C,BUN and Scr positive rate 58%,18.9% and 12.7%,respectively,by the chi-square test,Cys-C test positive rate and serum BUN,Scr,comparative differences are significant(P<0.01).Conclusion:Cys-C can be used in the diagnosis of diabetic nephropathy,and to some extent the degree of renal damage in patients with type 2diabetic nephropathy,Cys-C in type 2diabetic nephropathy early have higher positive detection rate,has important significance to the diagnosis.
