摘要
目的:探讨高同型半胱氨酸血症对脑梗塞进展、梗塞面积有无影响,为临床对脑梗塞患者的预防与治疗提供理论依据。方法:收集2012年1月至2013年12月急性脑梗塞患者共126例,患者分为两组:高同型半胱氨酸血症组64例(Hyperhomocysteinemia group,HHcy≥10mmol/L,HHcy);正常同型半胱氨酸血症组62例(normal hyperhomocysteinemia,Hcy<10mmol/L,nHcy,62例),神经功能缺损评分依据NIHSS评分。比较HHcy组和nHcy组患者梗塞进展例数及脑梗塞面积大小。根据脑梗塞面积,脑梗塞分为腔隙性脑梗塞、中度面积脑梗塞及大面积脑梗塞。结果:HHcy组,64例脑梗塞患者中,发生进展型脑梗塞共25例,进展发生率为39.06%,nHcy组发生进展型脑梗塞共10例,进展发生率为16.13%,两组比较P<0.05,有统计学意义;脑梗塞面积比较:nHcy组以腔隙性脑梗塞为主,HHcy组以中度及大面积脑梗塞为主。结论:HHcy与脑梗塞关系密切相关,HHcy脑梗塞容易进展且脑梗塞面积大。
Objective:To analysis influence of hyperhomocysteinemia on progress and infarct size of cerebral infarction in acute cerebral infarction patients and to provide reliable theoretical basis for preventing and treating cerebral infarction.Methods:126patients with acute cerebral infarction were assigned into two groups:hyperhomocysteinemia group(HHcy group,64cases)and normal homocysteine group(nHcy group,62cases),the two groups scores of neurological deficit were evaluated by National Institutes of Health Stroke Scale(NIHSS).To compare cases of progress and infarct size of cerebral infarction in patients with acute infaction.Accoding to size of cerebral infarction,type of cerebral infarction is devided into lacunar infarction,moderate infarction and large infarction.Result:Of all 64 cases in HHcy group,25 caese were progressive cerebral infarction.The incident of progressive cerebral infarction in HHcy groupwas39.06%.in62 casesofnHcy group,ten cases were progressive cerebral infarction and the incident of progressive cerebral infarction was 16.13%.The statistic was significant between HHcy group and nHcy group,P<0.05.The patients in nHcy group were given priority to lacunar infarction.The percent of moderate infarction and large infarction cases in HHcy group than that in nHcy group.Conclusion:HHcy is closely related to the cerebral infarction.The serious condition of cerebral infarction with HHcy is easy to progressive,and the size of infarction in HHcy group is large.
