血气分析对心内直视手术患者围术期肺功能监测意义

EVALUATION OF BLOOD GAS ANALYSIS ON PULMONARY FUNCTION IN PATIENTS UNDERGOING OPEN HEART OPERATION BY CPB

目的 :分析体外循环手术围术期患者血气结果 ,进而通过血气分析对肺功能进行评价 ,指导临床治疗。方法 :选择直视心脏手术的患者 4 0例 ,分别于围术期不同时间 (术前、术后 2 4小时、术后 4 8小时、术后 72小时、术后 6天 )采集动脉血进行血气分析 ,以了解体外循环对肺功能的影响。结果 :体外循环对肺功能有较大的负面影响 ,并在术后短期有进展趋势 ,术后 3天开始逐渐恢复 ,但术后 6天仍未恢复术前水平。结论 :经体外循环心内直视手术后 ,患者肺功能明显下降 ,且术后短期内有进展趋势 ,加强术后呼吸管理 ,吸氧持续到术后

Objective:In order to discuss the effects of Cardio pulmonary Bypass (CPB) on pulmonary function perioperative in patients undergoing open heart operation.Methods:40 patients undergoing open heart operation by CPB were measured blood gas in different time perioperative.All patients were compared through self control,then analysize the results.Results:Pulmonary function decreased significantly after operation and less and less in 3 days,then restore slowly,wasnt normal till 6 days after operation.Conclusion:Pulmonary function decreased significantly after extracorporal circulation,O2 inhalation and some airway protective ways are helpful for restoring health quickly.
