2001年青岛市农村居民疾病的经济负担及其影响因素分析

Studies on direct and indirect economic burden of disease and related factor in countryside of Qingdao city in 2001

目的 研究农村居民疾病的直接和间接经济负担 ,探讨影响农村居民疾病的经济负担的因素。方法 应用二步模型法和人力资本法对疾病的经济负担进行估算。结果 2 0 0 1年青岛市农村 3 3 5 9名调查人口的疾病总经济负担为 3 0 72 2 2 5元 ,其中慢性非传染性疾病占 65 .92 % ,传染病、妇科与围产期疾病占 2 4.2 5 % ,伤害占 9.83 % ;直接经济负担为 15 99619元 ( 5 2 .0 7% ) ,间接经济负担为1472 60 6元 ( 47.93 % ) ;平均每个居民的经济负担为 914元。不同年龄别之间的两周病伤经济负担和年住院经济负担均有显著的统计学差异 ,其中以 3 0～ 3 9岁和 60岁以上年龄组最高。男性的住院经济负担高于女性。有残疾状态的患者与无残疾状态的患者的经济负担平均值分别为 3 0 70元和 680元 ,两者之间差异有显著性 (P <0 .0 0 1)。影响农村居民疾病经济负担的重要因素为患有慢性病、年龄增大、有残疾状态、离婚丧偶等。结论 根据不同年龄段的疾病谱制订相应策略 。

Objective To study the condition of economic burden of disease in the countryside and to explore the related factors. Methods Human capital method and two-step method were used in the calculation of economic burden of disease. Results The total economic burben of disease among 3 359 persons was 3 072 225 Yuan. Noncommunicable conditions were accounted for 62.95%, while communicable disease,maternal and perinatal conditions accouted for 24.25%, and injury accouted for 9.83% respectively. The direct economic burden of disease was 1 559 619 Yuan and the indirect economic burden of disease was 1 472 606 Yuan. The economic burden of disease for each person was 914 Yuan. The equal burden of disease among patients with disability and without disability were 3 070 Yuan and 680 Yuan respectively ( P< 0.001). There was significant difference among diffirent age groups.The influencing factors were found to include having noncommunicable disease, age, disability and the condition of marriage. Conclusion Corresponding policy to cope with conditions of different age groups needs to be developed to reduce the economic burden of disease in the countryside.
