摘要 目的 探讨1 8F 脱氧葡萄糖 (FDG)hPET CT显像对肺部病变的诊断价值。方法 对 5 7例经病理检查或临床及其他影像学诊断的肺部病变患者进行胸部1 8F FDGhPET CT显像 ,勾划肺内病灶感兴趣区 (ROI) ,在对侧相应位置复制相同大小ROI,测定计数得靶 非靶比值 (T N)和病灶大小 ,并对良恶性病变的T N比值进行比较。结果  44个肺内病灶中1 8F FDGHDET CT显像示 2 7个为恶性 ,17个为良性 ,其T N比值分别为 8.49± 8.80和 3 .40± 1.92 (P <0 .0 1)。采用受检者工作特征 (ROC)曲线分析 ,以T N比值为 3 .5作为判断肺部恶性病变的标准 ,1 8F FDGhPET CT诊断的灵敏度为74 1% ,特异性为 76.5 % ,准确性为 75 .0 %。结论 1 8F FDGhPET Objective To evaluate diagnostic ability of 18F-FDG hPET/CT imaging in patients with pulmonary nodules. Methods Fifty-seven patients with known or suspected malignant pulmonary nodules underwent thoracic 18F-FDG hPET/CT imaging at 60 min after the intravenous injection of 222~296 MBq 18F-FDG. Regions of interest (ROI) were placed over the nodules (T) and contralateral normal lung tissue (N) on coronal view, and T/N value was calculated. Based on the receiver operation characteristic curve the sensitivity, specificity and accuracy of T/N value in differentiation of malignant from benign lesions was evaluated. Results Surgical pathology and follow-up revealed 27 malignant nodules and 17 benign lesions. The T/N value of malignant nodules was 8.49±8.80, and the value of the benign lesions was 3.40±1.92, significantly different from each other (t=2.892, P<0.01). The sensitivity, specificity and accuracy in differentiating malignant and benign lesions were 74.1%, 76 5% and 75.0%, respectively. Conclusion This preliminary clinical study indicates that 18F-FDG hPET/CT imaging is useful in localization and differentiation of pulmonary malignant nodules from benign lesions.
